| Description | Title 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, required that activities affecting quality be accomplished in accordance with written procedures. Diablo Canyon Power Plant Procedure OP1 L-7, Plant Stabilization Following Reactor Trip, Section 6.9.2 required that the bypass valves for the main steam isolation valves be opened to provide a drain path to the installed steam traps. Contrary to this, from April 25-27, 2012, the licensee failed to open the bypass valves while aligning the steam system following a reactor shutdown. On April 27, 2012, plant operators identified that the steam plant had not in been in the correct valve lineup. This condition could have resulted in the loss of turbine-driven auxiliary feedwater pump safety function due to accumulation of condensation in the steam supply line. This finding was of very low safety significance (Green) because the finding was not a design or qualification deficiency confirmed not to result in loss of operability or functionality, did not represent a loss of system safety function, and did not screen as potentially risk significant due to a seismic, flooding or severe weather initiating event. Pacific Gas and Electric entered the issue into the corrective action program as Notification 50477779. |
