| Title | Multiple ESFAS Steam Line Pressure Channel Modules Inoperable Due to Inadequate Calibration Instructions |
| Description | A finding of very low safety significance and associated non-cited violation of 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, was self-revealed for the failure to have adequate maintenance procedures for calibrating the engineered safety features actuation system steam line pressure dynamic compensation modules. Specifically, since the basis calculation for determining the settings of the lead/lag values for the modules did not address dynamic settings, and the proceduralized tolerances were too restrictive, the calibration instructions were inadequate to ensure the modules ability to perform in accordance with technical specification requirements. Upon discovery, the licensee entered the issue into its corrective action program and performed an apparent cause evaluation that documented a number of planned program and procedural enhancements. The finding was more than minor because it is associated with the equipment performance attribute of the Mitigating Systems Cornerstone and affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. The finding was determined to be of very low safety significance because there was no design deficiency, no actual loss of safety function, no single train loss of safety function for greater than the technical specification allowed outage time, and no risk due to external events. The finding does not have a cross-cutting aspect because the performance deficiency occurred outside of the 3-year window considered to be representative of present performance. |
