| Description | 10 CFR Part 50, Appendix B, Criterion V, requires, in part, that activities affecting quality shall be prescribed by documented instructions or procedures of a type appropriate to the circumstances. Contrary to the above, on September 21, 2010, an unplanned reactor trip occurred while the quarterly surveillance for the Channel II High Pressurizer Pressure Protection Loop (P-4-456) was in progress. The licensee determined the root cause to be inadequate inspection and installation criteria used for ELCO connectors because acceptance criteria and method of verification where not addressed by procedures. The issue was screened to be of very low safety significance (Green). When identified, the licensee took corrective actions to add ELCO connector inspection requirements to a plant procedure and conduct formal training for maintenance personnel to properly inspect and mate connectors. The issue was documented in AR 00581322. Because the licensee identified the issue and documented it into their corrective action program, and because the finding is of very low safety significance, this violation is being treated as a licensee identified NCV consistent with the NRC Enforcement Policy. |
