Dear Mr. Engelken:

This letter describes a reportable occurrence invo~ ving the failure to perform certain of the surveillance tests required by Amendment 58 to the Provisional Operating License ib. DPR-13, Appendix A, Technical Specifications. This submittal fulfills the requirement for a 30-day report and LER in accordance with Section 6.9.2.b(3) of Appendix A to the Provisional Operating License.

The following monthly Channel Tests were not conducted as required for Modes 1, 2, 3, and 4 :

Section 4.1.6. A.1; PORV Channel Test These surveillance tests were inadvertently omitted during a period of operation commencing December 15,1981 (the effective date of Amendment

58) and ending February 28, 1982 when the plant entered Mode 5.

R.H. Engelken April 16,1982 Prior to the implementation of the Technical Specification change, a multi-department review of Amendment 58 was accomplished in order to assign responsibilities for compliance. This review resulted in a memorandum being issued on November 24, 1981 which specified departmental responsibility for compliance with each section of Amendment 58. However, the complexity of this amedment and misunderstanding concerning responsibilities for some of the detailed sub-sections resulted in the tests referenced above being overlooked. The failure to perform the required channel tests was discovered during a follow-up review by Station personnel.

Administrative controls are being revised to improve inter-departmental communication and coordination during license amendment reviews. A com-prehensive multi-departmental review of Amendment 58 is presently being conducted. This will ensure specific responsibility assignment, id-entification of implementing procedures, and establishment of schedules to ensure timely compliance.

The aforementioned channel tests will be completed prior to entering Mode 4 at the conclusion of the current outage.

There was no release of radiation, and no impact on public health and safety associated with this event.
