Title | Licensee-Identified Violation |
Description | TS limiting condition for operation (LCO) 3.3.3.1, Condition F, Post Accident Monitoring (PAM) Instrumentation, states in part , with the DWHRRMs inoperable, a Special Report shall be submitted to the Commission within the next 14 days. Contrary to the above, the licensee failed to identify the inoperability of the DWHRRMs after the NRC Information Notice 97 -45 Supplement 1 was issued. In particular, the DWHRRMs signals cables are susceptible to thermally induced currents which can degrade the accuracy of DWHRRMs . The required action of LCO 3.3.3.1 , action F, was not perf ormed from 1998 until December 5, 2016. Using IMC 0609, Appendix B, Emergency Preparedness Significance Determination Process, inspectors determined that this violation was of very low safety significance (Green) because the finding is related to Emergenc y Preparedness Requirements that are not associated with a planning standard function (e.g.,10 CFR 50.54(q), 10 CFR 50.54(t), and requirements in Appendix E to 10 CFR Part 50 that do not support a planning standard function ). Other parameters could be used to validate the indications from the DWHRRMs . The corrective action is to restore the monitors to operable. This issue was documented in the licensees CAP as NCR 2066681 |
