| Description | Title 10 CFR 50.65(a)(1), Requirements for monitoring the effectiveness of maintenance at nuclear power plants, requires, in part, that holders of an operating license shall monitor the performance or conditions of structures, systems, or components within the scope of the monitoring program against licensee established goals in a manner sufficient to provide reasonable assurance that such structures, systems, or components are capable of fulfilling their intended safety function. Contrary to the above, on November 6, 2013, the licensee identified that they failed to establish goals in a manner sufficient to provide reasonable assurance that structures, systems, or components were capable of fulfilling their intended safety function. Specifically, the licensee failed to establish goals for the main condenser when it was placed in an (a)(1) status. This performance deficiency was more than minor, and therefore a finding, because it was associated with the equipment performance attribute of the Initiating Events Cornerstone, and affected the associated cornerstone objective to limit the likelihood of events that upset plant stability and challenge critical safety functions during shutdown as well as power operations. Using Inspection Manual Chapter 0609, Appendix A, The Significance Determination Process (SDP) For Findings At-Power, inspector
determined that the finding was of very low safety significance (Green) because th finding did not cause a reactor trip and the loss of mitigation equipment relied upon t transition the plant from the onset of the trip to a stable shutdown condition. This issu was entered into the licensees corrective action program as Condition Report CR-CNS-2013-07967 for resolution. |
