WALKDOWN CHECKLIST FORM Sheet I of 5 IP3 Status: YEI NOIl U-Seismic Walkdown Checklist (SWC) SWELl-016 Equipment ID No. 52/RTB & 52/RTA Equip. Class 1 2 Equipment Description RX TRIP SWITCHGEAR RTA, RTB Location:

Bldg. CB Floor El. 33-O" Room, Area Manufacturer, Model, Etc. (optional but recommended)

Instructions for Completing Checklist This checklist may be used to document the results of the Seismic Walkdown of an item of equipment on the SWEL. The space below each of the following questions may be used to record the results of judgments and findings.

Additional space is provided at the end of this checklist for documenting other comments.R1 Anchorage 1. Is theanchorage configuration verification required (i.e., is the item one of the 50% of SWEL items requiring such verification)?

Not part of anchorage checks.2. Is the anchorage free of bent, broken, missing or loose hardware? The bottom cubicles were opened in 3R1 7. The anchorage is free of bent, broken, missing or loose hardware 3. Is the anchorage free of corrosion that is more than mild surface oxidation?

The bottom cubicles were opened in 3R1 7. The anchorage is free of corrosion that is more than mild surface oxidation.

4. Is the anchorage free of visible cracks in the concrete near the anchors? There is a crack on the coating near the angle iron attachment; however there is no crack on the concrete.

The anchorage is free of visible cracks in the concrete near the anchors.Enter the equipment class name from EPRI 1025286, Appendix B: Classes of Equipment.

8. Are overhead equipment, distribution systems, ceiling tiles and lighting, and masonry block walls not likely to collapse onto the equipment?

Masonry block walls were addressed by the IE 80-11 license commitment program.9. Do attached lines have adequate flexibility to avoid damage?10. 11. Have you looked for and found no other seismic conditions that could YE Nil UI--adversely affect the safety functions of the equipment?

The following conditions were observed but they are not adverse to the equipment functionality:

1. Fuse blocks in the cubicle above RX breaker A and Bypass breaker A cubicles is lying on the shelf during the breaker outage. This will be placed back to on position once the breakers are energized.
2. The labels for the RX breaker A and Bypass breaker A cubicle have only one screw instead of two. The label is very light and 1 screw will be sufficient to hold it in place during a DBE.3. The back panels for the breakers all have two top bolts and one right hand side bottom bolt. This condition seems to be generic because 1P2 has the same condition.

Three bolts have sufficient capacity to hold the panel in place during a DBE.4. There is a slight, localized surface corrosion on the left lever arm of the breaker.Comments (Additional pages may be added as necessary)

Since Operations advised us that this equipment cannot be opened during an operating cycle, and that It is only allowed to be opened during anoutage, the RX Trip Switchgear RTA, RTB were inspected during the 3R1 7 Outage. The equipment was looked at from the front and back during 3R1 7.
