Therequiredsurveillance testwastoperformachannelcheckoftheAveragePowerRangeMonitor(APRM)PG]flowsignaltoverifyoperability oftheAPRMflowbiasedsimulated thermalpowerhighrangetripchannels.

Thetestisrequiredtobeperformed onadailybasiswheninMode1,whichhadbeenenteredat0914hoursonJune20,1996.Therequiredtesthadbeencompleted onJune22,1996,andagainonJune24,1996.However,thetesthadnotbeenperformed onJune21or23,resulting inaviolation ofTechnical Specification 3.3.1.min:Therequiredsurveillance testhadbeencompleted approximately 7.3hoursearlier,at0139hoursonJune24,1996,demonstrating thatflowratedatawasbeingcorrectly receivedandutilizedbytheAPRMsystem.Consequently, noimmediate corrective actionwasnecessary.

mRa&nm:Therootcauseofthiseventwasreactorplantstartupandsurveillance procedures thatwereincorrect oromittedrelevantinformation.

Thesurveillance procedure beingusedcontainsthreeseparatepartscoveringthreedifferent surveillance requirements:

jetpumptP]operability, RRCloopflowbalance,andtheAveragePowerRangeMonitorflowchannelcheck.The"Purpose" sectionofthissurveillance procedure includedthestatement, "Thissurveillance isperformed within24hoursofthermalpowerexceeding 25%ofratedthermalpower,inLEtlessthanorequalto]4hoursafteranidlerecirculation loopisreturnedtooperation, anddailythereafter whenthereactorisinoperational conditions 1or2.Thatisanincorrect statement sinceitdoesnotreflecttherequirement ofTechnical Specification 3.3.1toperformtheAPRMfiowsignalchannelcheckonadailybasiswheninMode1.Theprocedure governing reactorplantstartupstatedthattheparticular surveillance procedure wasrequiredafterreaching25%reactorpower,andtherefore alsodidnotaddresstheTechnical Specification requirement toperformtheAPRMflowsignalchannelcheckonadailybasisinMode1.Thesurveillance procedure governing shiftanddailyinstrument checksdidnotidentifytheAPRMflowsignalchannelcheckasbeingarequiredactionduringMode1~

Theeventhadnosafetysignificance.

Therequiredsurveillances hadbeenperformed onJune22and24verifying operability oftheAPRMflowsignalinput,andwereperformed asrequiredbyTechnical Specifications afterJune24.TheAPRMhighneutrontripfunctionwasfunctional duringthisentireperiod,assuringthattheReactorProtection System[JC]wouldscramthereactorinthe"eventofanunexpected neutronspike.LicenseeEventReport96-003-00 identified aneventinvolving asurveillance testinadvertently omittedduetothesameinadequate procedures duringaplantstartup.ThiseventhadoccurredonJune15,1996.Ifcorrective actionsforthispreviouseventhadbeencompleted priortotheLER96-005-00 event,theycouldhaveaffordedtheopportunity toidentifyandcorrecttheprocedural deficiencies thatledtoLER96-005-00.
