TheauditteamassessedspecificareasasrequiredbyNUREG0654,"Criteria forPreparation andEvaluation ofRadiological Emergency ResponsePlansandPreparedness inSupportofNuclearPowerPlants."Theauditteam'seffortswerecomplimented bytheparticipation fromthreeoutsidespecialists.

AnEPspecialist fromWolfCreekNuclearOperating StationandanEPtrainingspecialist fromDiabloCanyonNuclearFacilityservedtobroadenth'eoverallexperience baseoftheteam.Aswell,arepresentative fromtheOregonStateOfficeofNuclearEnergyprovidedstateagencyexpertise.

Theteam'sacknowledgement andthanksareextendedtotheseindividuals fortheiroverallcontribution.

Theresultsoftheauditfoundthatactivities associated withtheWNP-2Emergency Preparedness Programareimplemented inaccordance withgoverning regulations andarecapableofprotecting thehealthandsafety"ofthepublicintheeventofanemergency.

Theteamnotedimprovements inEmergency ResponseOrganization (ERO)teamstaffinglevelsandoverallmaintenance ofpersonnel qualifications.

Also,thecurrentannualrefresher-trainingformatwasrecognized asapositiveelement.TheEmergency Preparedness Organization demonstrates strongprogramownership, isproactive intheidentification andresolution ofproblems, andhasestablished effective communication withoff-siteagencypersonnel.

AlthoughtheEPprogramisstrong,someareasforimprovement wereidentified.

TheteamfoundtheEROdrillperformance

adequate, however,'inconsistencies indrillperformance werenoted.Also,somerecurring areasforimprovement identified indrillplayhavenotshownimproving trends.Additionally, theteamdiscovered someminorprocedural discrepancies primarily associated with.theimplementing procedures.

Clarification ofourLicenseBasisDocument(LBD)commitments relativetothestation's meteorological capabilities isrequired.

Overall,sevenProblemEvaluation Requests(PERs)wereinitiated toaddresstheseissues.NineQualityRecommendations wereofferedtocorrectinappreciable errorsintheimplementing procedures orassuggested programenhancements.

MPORTDETAILSSECTION1.0Emergency PlanandImplementing Pr0cedures Areviewwasconducted todetermine iftheEmergency Plan(E-Plan)Implementing Procedures (Volume13ofthePlantProcedure Manual)satisfytherequirements oftheWNP-2E-Plan.Additionally, thereviewverifiedthattherequirements of10CFR50AppendixEandNUREG0654werecontained intheE-Plan.Thereviewconcluded thattheWNP-2Emergency Preparedness Programsatisfies theregulatory requirements.

TheE-Planrequiresaninternalreviewoftheplanandanindependent auditoftheplanandimplementing procedures every12months.Anevaluation ofthereviewprocessdetermined thattheE-Planhasbeenupdatedasneededfortheperiodsreviewed, E-Planchanges(twoin1995,twoin1996,onein1997,andtwoin1998)wereverifiedfromRevision15tocurrentRevision21.Thesechangesareinaccordance withtheguidanceinPlantProcedure Manual(PPM)1.3.43,"Licensing BasisImpactDetermination,"

regarding changestotheE-Plan.Areviewofpreviousauditreportsconcluded thatappropriate areaswereassessed, theauditswereindependent ofEmergency Preparedness (EP)personnel, andtheauditswereperformed within12months.AppendixEof10CFR50states;"Licensees whoareauthorized tooperateanuclearpowerfacilityshallsubmitanychangestotheE-Planorprocedures totheCommission, asspecified inSection50.4,within30daysofsuchchanges."

Theauditteamfoundrecordsoftransmittal QualityAuditReptttt299-007~forallreviewedE-PlanchangesinPlantFileswiththeexception ofRevision18.Althoughrelatedcorrespondence showedthattheNRChadreceivedthisrevisionoftheE-Plan,norecordoftransmittal orassignedLicensing numbercouldbefound.Theauditteamreviewedthesubmittal processasdescribed intheAdministrative HandbookandAdministrative ServiceInstructions (ADSQ,"Manual/Revision PackageProcessing andDistribution."

Theteamfoundthatthecurrentprocessdescribed forsubmittal oftheE-Planisnotbeingfollowed.

Thesesubmittal discrepancies resultedinaQualityinitiated PER.QualityFinding(PER299-0517)

Currentprocessforsubmitting revisedE-PlantotheNuclearRegulatory Commission (NRCJisnotbeingfollowed.

UnabletolocateRevision18submittal letterinPlantFiles,AreviewoftheWNP-2E-Planfoundthat,althoughtheE-Plandiscusses theorganizations requiredtorespondtoanemergency, itdoesnotstatewhenafacilityisactivated, whatconstitutes activation orwhatisnecessary todeclarethefacilityactivated.

Theimplementing procedures directthemanagerstoactivatethecenterswhenthemainresponsibilities canbeperformed, buttheE-Plandoesnotdescribewhattheseresponsibilities are.Non-specific information onfacilityactivation couldresultinareduction ineffectiveness ifimplementing procedures arechanged.Aprocedure changetoreducethenumberofpersonsrequiredtoactivatethefacility, whichwouldnotaffecttheE-Planaswritten,couldbeviewedasareduction ineffectiveness.

Allexercises aresubjecttotheapprovaloftheManager,Regulatory Affairs,thePlantGeneralManager,andtheVicePresident, NuclearOperations."

PPM13.14.8,directstheCorporate Emergency Preparedness, SafetyandHealthOfficertoscheduleandcoordinate theannualdrill/exercise programwiththefollowing:

SupplySystemWNP-2PlantGeneralManager;VicePresident, NuclearOperations; andVicePresident, Operations Support/Public Information Officer.Additionally, Section8.6states,"TheCorporate EPOfficerwillmakeeveryefforttonotifytheoffsiteemergency responseorganizations andagenciesatleast180daysinadvanceofthescheduled dateofthedrillorexercise."

PPM13.14.8,doesnotprovidethissamerequirement.

Theteamfoundevidencethisnotification wasbeingmadeasrequiredbytheE-Plan,however,theprocesswasinformalandnotadequately described intheimplementing procedure.

I'ollowing discussions withtheauditteamtheEPstaffdocumented theseprocedural discrepancies inaPER.QualityFinding(PER299-0442)

E-PlanImplementing Procedure 13.14.8doesnotaccurately reflectrequirements identiJtedintheE-Plan.TheauditteamreviewedPPM13.14.9,"Emergency ProgramMaintenance,"

whichidentifies theactivities necessary tomaintainthecurrentemergency preparedness programinaccordance withcommitments madeintheE-Plan.Attachment 5.1,part1.bstates"DOEiscontacted earlyinthereview/revision process(E-Plan)andthattheircommentsaresolicited

'andconsidered forinputintotheprocess.Documentthisportionofthereview."Theteamcouldnotfinddocumented evidenceofthisreview.Thisissuewasdiscussed withEPstaff,whichreviewedtheprocedure andidentified additional discrepancies regarding performance ofrequiredreviews.EPdocumented theseissuesintheinitiation ofPER299-0443.

5 QuetitAuditReport299-007~QualityFinding(PER299-0443)

Procedural requirements inPPM13.14.9relatingtotheE-Planreviewhavenotbeenmet.SECTION2.0Emergency ResponseOrganimtion TrainingAreviewoftheWNP-2E-Plan(Rev.21),theQualification Directory, theEmergency Preparedness TrainingCourseCatalog,andtheSite-Wide Procedure onEROconcluded thatappropriate trainingisofferedforeachidentified EROposition.

Thetrainingincludesfacility/ERO overviewlessonsaswellasgeneraltrainingrequirements forallEROpositions.

Additionally, positionspecificandprocesstrainingisofferedforsomeEROmembers.Therearealsoprovisions forannualrefresher training.

Annualrequalification trainingisprovidedforallEROpersonnel.

Thistrainingisprovidedbyseveralmeansincluding mail-outmaterial, classroom traininganddrills.Forannualrefresher

training, theentireteamisbroughtintotheauditorium andpresented withinformation regarding hardwareandprocedure changes,andlessonslearnedfromin-houseandindustryoperating experiences.

Thentheteamsbreakoutintotheirrespective facilities andareaskedtointroduce themselves, explaintheirresponsibilities inthecenter,describetheirprimaryinterfaces, andidentifythesupporttheyexpectfromothermembersoftheteam.Duringinterviews withEROmembers,theauditteamreceivedoverwhelming positivefeedbackofrecentrefresher training.

Theauditteamconsidered theformatfortheannualteamrequalification classroom trainingtobeapositiveprogramelement,EPmakesaconcerted efforttohaveeachnewEROmemberobservehis/herpositionfunction"duringtheperformance ofadrill.Thenwhenappropriate, thenewmemberwillperformthepositionfunctions duringadrillundertheobservation ofanincumbent "coach".Then,inmostcases,newmembersarereadytoperformtheirpositiondutiesontheirown.'Basedondiscussions withpersonnel, theteamconcluded theinitialtrainingprovidedbyEPdoesanadequatejobofpresenting thebackground information andthetoolsnecessary toaidnewmembersinperforming theirduties.However,mostmembersdidn'tfeelcompletely comfortable intheir'positions untiltheywereabletoactuallyparticipate ontheirowninadrill.Thisisreasonable andexpected.

-TrainingsubjectsforEROpersonnel arecurrently selectedbasedontheneedsoftheindividual

position, asdocumented intheQualifications Directory andtheEPTrainingCourseCatalog.TheSite-Wide Procedure SWP-EPP-01, "Emergency ResponseQualifications andTraining,"

However,theoriginaltaskanalysis/training databasehasnotbeenmaintained.

Therefore, theauditteammakesthefollowing recommendation:

QualityAuditReport299-007CQualityRecommendation AU299-007-D Assignedto:TCMessersmith DueDate:June14,1999ReviseProgramContentDescription asdescribed inprocedure SWP-EPP-Ol toaccurately reflectcurrentbasisforEPTrainingProgram.ThepositionofTrendingForecaster hasrecentlybeenaddedtotheEROroster.Trendingforecasters trainedinSevereAccidentManagement Guidelines (SAMG)areexpectedtomonitorandtrendkeyplantparameters duringsevereaccidentsituations andapplytheGuidelines asconditions warrant.Thetrendingforecaster workswithotherSAMGtrainedindividuals, typically theengineering personnel andthefacilitymanagerwithintheTechnical SupportCenter(TSC).Theauditteamfoundthroughinterviews, thatpersonnel werenotconfident thatthetrainingtheyhadreceivedwouldassuretheirabilitytoperformtheirdutiesifSAMGswererequired, Individuals whohadstudiedSAMGsinOperations trainingexpressed confidence intheirabilities.

Ofthefourassignedtrendingforecasters, onlyonehasparticipated inadrillwheretherehasbeenatransition outofEmergency Operations Procedures intoSAMGs.Asaresulttheteammakesthefollowing recommendation:

Theauditteamobservedperformance oftheTeamCandAtrainingdrillsconducted February2,andFebruary23,1999,respectively.

Auditmemberswerestationed attheEmergency OffsiteFacility(EOF),Technical SupportCenter(TSC),Operations SupportCenter(OSC),andtheControlRoom(simulator).

Theauditteamalsoobservedthepost-drill critiqueateachlocation.

Inbothobserveddrills,theparticipants mettheobjectives ofthedrill.Specifically, thePlantwasbroughttoasafecondition, fueldamagewasmitigated, andthepublic'shealthandsafetywasprotected.

Theoverallassessment ofbothdrillswasthatteamperformance wasadequate.

However,theemergency organization hasnotestablished apre-designated organization ofcontrollers andevaluators tosupportdrillperformance.

Currently, individuals assignedasdrillcontrollers andevaluators areobtainedbyrequesting volunteers orlastminutedraftingofpersonnel.

Thisresultsininconsistent set-upandconductofthedrillitself.Thecontrollers areexpectedtocoachplayersandkeeptheprogressofthedrillontrack.Onseveraloccasions duringdrillplay,itwasnotedthecontrollers failedtocontrolthedrill,whichaffectedtheperformance oftheteam.Oneexample,notedduringtheTeam"A"drill,occurredwhenacontroller toldafieldteamthattheStartupTransformer wasbackinservice.Thisinformation wastransferred tothecontrolroom.Sincethedrillscenariohadnotactuallyreturnedthetransformer toservice,therewasconfusion inthecontrolroomanddelaysinthefieldteamactivities.

Themiscommunication wasultimately corrected bytheOSCcontroller.

~QuoitAuditReport299-007Controllers arealsoexpectedtoevaluatedrillplayandplayerperformance.

Theteamnotedthecontrollers experienced difficulty incontrolling whileevaluating.

Theircommentswereinconsistent andinsomecasesincomplete.

'Thishasresultedinlostopportunities forimprovement.

Thisisanissuethatwasidentified inthe1998E-PlanSelf-Assessment.

AlthoughanEPimprovement initiative actionhadbeenestablished, EPrecognized theissuewarranted moreimmediate attention.

TheEPstaffinitiated thefollowing PER:QualityFinding(PER299-0445)

Thelackofapre-designated organization ofcontrollers andevaluators resultsininconsistent drillconductandinconsistent andincomplete evaluations.

DrillReportsareissuedshortlyafterconductofthedrill.Thesereportsareappropriately detailedandcapturemostofthepositiveandnegativeattributes ofdrillperformance.

AfterActionReportcommentsandcorrective actionsaregenerated asaresultofthedrill.Thesereportsaredeveloped basedontheobservations oftheevaluators, inputfromotherobservers, andfeedbackfromdrillparticipants.

ThedrillreportsissuedfortheTeamCandTeamBdrillswerereviewed.

Theauditteamnotedsimilarareasforimprovement duringobservations ofTeamAandCperformance.

ManyoftheissuesdetailedintheDrillReportsforTeamsBandCwerediscussed inthetrainingsessionforTeamA.However,thesameproblemswereevidentduringtheconductofthedrill.Thefollowing havebeenrecurring areasforimprovement:

~Three-way communication

~DispatchofOSCrepairteams~ReleaseofEquipment Operators totheOSC~CommandandControl~Establishment ofrepairteampriorities Thelackofimprovement fromdrilltodrillisaconcern.Asaresult,theEPstaffinitiated thefollowing PER:QualityHnding(PER299-0517)

Areasforimprovement identified duringEROtrainingdrillsarenotshowingimproving trends.TheEmergency Preparedness ManageristheleaderofWNP-2'sContingency PlanforYear2000(Y2K).Currently, aRemediation Committee isidentifying internalequipment andexternaleventsthatrequiresometypeofovertaction(e.g.,reboot,andreset)tocontinueitsfunction.

TheContingency Managerhasstatedthatadesignated EROteamwillbeoncall,asusual,shouldsomeunexpected planteventtakeplace.However,atthispoint,thereisnointention toactivateanyemergency centerspecifically forthepurposeofaddressing Y2Kissues.Thenotification systemandpagershavebeenidentified as"Y2Kinert."Thismeansthatthe2000rolloverwillhavenoeffectonthesystem.Interviews withEPpersonnel about QualityAuditReport299-007Y2Kconcernsforthedoseprotection computersystemsfoundthatoneproblemwaspreviously identified.

Thisissuehasbeenaddressed andacontingency planisinplace.SECTION4.0Equipment andFacilities Theauditteamassessedwhetheremergency facilities andequipment identified byNUREG0696,"Functional CriteriaforEmergency ResponseFacilities" described intheFinalSafetyAnalysisReport(FSAR)havebeenadequately providedandmaintained.

Thisassessment determined thatsufficient equipment andfacilities areinplace'osupportemergency response.

TheteamvisitedallEROcentersanddetermined thatemergency responsefacilities arewellequippedforemergencies.

However,oneconcernregarding thevariouscomputers usedtosupportEROfunctions wasidentified.

Problemswithcomputers becameevidentduringthedrillheldFebruary23,1999.Whileallofthecomputers hadbeensuccessfully testedjusttheweekbefore,atleastthreeinvariouscentershadsomedifficulty inbootinguportransmitting information.

Themajorityofcomputers areolderandextremely slow.Inafollow-up bytheauditteamtothisconcern,EPexplained thatthecomputers areslatedforreplacement whenthePRIMEcomputersystemisreplaced.

Theseactionsarescheduled forlaterthisyear.Nofurtheractioniswarranted.

EPIP13.14.4,"Emergency Equipment,"

listsalloftheequipment usedinthevariousEROcenters,requirements forpreventive maintenance orverification ofequipment operability andavailable forms.Theseactivities reference PASSPORTworktasks.However,manyofthosetaskshavebeendeletedandthePASSPORTscreendoesnotexplainhowthetask'sperformance isbeingtracked.Examplesofsuchtasksarethosethatcheckemergency kitsatthelocalareahospitals.

Thesetaskswereinitially assignedPASSPORTactivitycodesKADLECEMERG.KlT, KENNEWICK EMERG.KIT andLOURDESEMERG.KIT.

Thesesameactivities arecurrently beingtrackedonPTLas153967,153968and153969respectively.

Asaresultofthisdisparity, thefollowing PERwasissued:QualityFinding(PER299-0393)

requiresthatagreements withoff-siteagenciesbereviewedannually, withthelatestrevisionreviewedbystateandlocalgovernment representatives.

Theauditincorporated areviewoftheestablished agreements betweentheSupplySystemandoutsideorganizations

'forsupportofradiological emergencies atWNP-2.Lettersofagreement werereviewedtoevaluatetheircompleteness andconsistency withtheWNP-2E-Plan.Eachoftheagreements wasfoundtobeconsistent withtheWNP-2E-Planandgenerally complete.

Afewminor QualityAuditReport299-007~discrepancies werebroughttotheattention oftheemergency preparedness group.Alloftheagreements werefoundtobecurrent.TheBentonandFranklinCountyEmergency Directors wereinterviewed toobtainanunderstanding oftheworkingrelationship betweentheSupplySystemandcounties'mergency preparedness organizations.

Fromthesediscussions, itwasconcluded thattheSupplySystememergency preparedness groupmaintains aneffective relationship insupporting theseoff-siteorganizations.

WNP-2computer-based doseprojection programsconsistoftheEmergency DoseProjection System(EDPS),QuickEmergency DoseProjection System(QEDPS),andtheBackupEmergency DoseProjection System(BEDPS).Thesearestand-alone systems,whichhavealternate powersuppliesandallowfordataacquisition intheeventofapowerlossduringanemergency.

Theauditteamverifiedthatthesesystemswereinplace,available, andcapableofproviding sufficient doseprojections inatimelymanner.Itwasalsonotedthatthesesystemsweresimilartooff-sitedoseprojection systemswithnoobviousdifferences indoseprojection data.Theauditteamrevieweddoseprojections procedures 13.8.1,"Emergency DoseProjection Procedure,"
