DuetospecificcircumstancescurrentlyexistingatD.C.CookNuclearPlantUnit2,wearerequestinganemergencychangetothesubjectTechnicalSpecification(T/S).Specifically,theT/Sstates:"Asaminimum,twosourcerangeneutronfluxmonitorsshallbeoperating,eachwithcontinuousvisualindicationinthecontrolroomandonewithaudibleindicationinthecontainmentandcontrolroom.ACTION:Withthezeguizementsoftheahovespecificationnotsatisfied,immediatelysuspendallopezationsinvolvingCOREALTERATIONSorpositivereactivitychanges..."ThecurrentcircumstancesatthePlantare:(1)OnFriday,April20,1984,duringtheUnit2refuelingactivities,oneofthetwosourcerangeneutronfluxmonitorsbeganbehavingerratically,renderingitinoperable.(2)Wehaveproceededtopartiallydewatertherefuelingcavitytomakerepairstotheinoperablemonitor.Wehavereplacedthedetectorandarestillunabletoachievesuccessfuloperation.(3)Defueling(COREALTERATIONS)activitiesweresuspendedwithallbut30fuelassembliesremoved.(4)Twooftheremaining30fuelassembliescontainsourcesforstartupofthereactor.FifteenassembliesarenearoperabledetectorN31andfifteenassembliesarenearinoperabledetectorN32.(5)Continueddelaystofixtheinoperabledetectorpriortocompletionofdefuelingwoulddelaystartupcommensuratelyfornoapparentsafetyreasons.8404300023840424PDRADOCK05000316DR IWp

~Mr.HaroldR.DentoAEP:NRC:0890~~Therefore,weareproposingthatT/S3.9.2beamendedtoallowdefuelingoftheremaining30assembliesduringthecurrentUnit2(1984)outage,withonlyoneOPERABLEsourcerangeneutronfluxmonitor.Basedonourreviewofthecurrentcoreconditions,wehaveconcludedthatthecoremultiplicationfactor(k-effective)willalwaysbereducedbytheremovaloffuelassembliesandthecommensuratereplacementof2000ppmboratedwater.Therefore,webelievetheremaining30fuelassembliescouldberemovedwithonlyoneOPERABLEsourcerangeneutronmonitorandnotcreatesignificanthazardsconsiderationasdefined10CFR50.92.Specifically,theproposedchangewillnotincreasetheprobabilityorconsequencesofapreviously-analyzedaccident.Thechangemayreduceinsomewayasafetymargin,butitisclearlywithintheacceptancecriteriaformaintainingthecoresubcritical.TheAttachmenttothislettercontainstheproposedrevisedT/Spage.Thechangeisindicatedbyverticalbarsdrawnontherighthandmarginofthepage.TheproposedchangehasbeenreviewedbythePlantNuclearSafetyReviewCommitteeandwillbereviewedbytheNuclearSafetyandDesignReviewCommitteeatitsnextscheduledmeeting.Pursuanttotherequirementsof10CFR50.91(b)(1),acopyofthisletteranditsattachmenthavebeentransmittedtoMr.R.C.CallenoftheMichiganPublicServiceCommission.WeinterpretthisapplicationforalicenseamendmenttoconstituteaClassIIIAmendmentasdefinedin10CFR170.22.Therefore,acheckintheamountof$4000.00willbeforwardedinthenearfutureaspaymentforprocessingtheaforementionedrequest.ThisdocumenthasbeenpreparedfollowingCorporateprocedureswhichincorporateareasonablesetofcontrolstoinsureitsaccuracyandcompletenesspriortosignaturebytheundersigned.Verytrulyyours,R.F.HeringVicePresidenAttachmentcc:JohnE.DolanW.G.Smith,Jr.-BridgmanR.C.CallenG.CharnoffE.R.Swanson,NRCResidentInspector-Bridgman elt>pIIII~P'II'f,CrJPI'II~I'$*I,',P'Ja~Pfl4'llII'4'ClwwJl,','Jrgp")III'=I~I~4IVI'44It~)*JI'IIrPlI"4'l~',I-pII'4II'I'P~~,p-4!"I1rfhI+Pw,I'1I'IIPw
