Attachment1toAEP:NRC:1170DPage2Also,Instrument14ofTable3.3-10requiresthat,asaminimum,onechannelpervalvebeoperable.Weareproposingtochangethistorequirethat,asaminimum,twooutofthreechannelstotalbeoperable.4.0DISCUSSIONSstemDescritionThereactorcoolantsystemisprotectedagainstoverpressurizationbycontrolandprotectivecircuits,suchasthepressurizerpressurehighreactortrip,andbythethreepower-operatedreliefandthreesafetyvalvesconnectedtothetopofthepressurizer.Uponopening,thesevalvesdischargesteamintothepressurizerrelieftank,whichcondensesandcollectsthevalveeffluent.Thissubmittaladdressesthemonitoringinstrumentationassociatedwiththepressurizersafetyvalvesandthepressurizerrelieftank.Thepurposeoftheacousticmonitorsistoprovidetheoperatorwithinformationregardingsafetyvalvepositionduringpost-accidentconditions.Twoindependentmonitoringsystemsexistthatalerttheoperatortothepassageofsteamthroughthesafetyvalvesduetovalvelift.Inoneoftheindependentmonitoringsystems,anacousticmonitor(QR-107A,B,andC)onthedischargeofeachsafetyvalvedetectsacousticvibrationsgeneratedfromthesteamflowingthroughthepressurizersafetyvalve,andactuatesanalarmandprovidesavisualdisplayofpercentageoffullflowinthecontrolroom.Duetothemagnitudeoftheacousticvibrationsandthesensitivityoftheinstrumentation,allthreemonitorswilldetecttheopeningofanyoneofthethreepressurizersafetyvalves.TheacousticmonitorswereaddedtomeettherequirementsofNUREG-0578,"TMI-2LessonsLearnedTaskForceStatusReportandShortTermRecommendations,"andNUREG-0737,"ClarificationofTMIActionPlanRequirements,"andareenvironmentallyqualified.Duringnormaloperationtheacousticmonitorsarenotusedtodetectpressurizersafetyvalveleakage.Intheotherindependentmonitoringsystem,atemperaturesensor(NTA-151,-152,-153)downstreamoftheacousticsensorgeneratesasignalthatactuatesacontrolroomalarmwhenatemperatureincreaseisexperiencedintheline,aswouldbethecaseifthepressurizersafetyvalvereleasedsteam.Theattachedsimplifieddiagramillustratesthisportionofthesystem.Indicationsofthetemperature,pressureandwaterlevelofthepressurizersafetyrelieftankarealsoprovidedinthecontrolroomandprovideadditionalindicationofpressurizersafetyvalveleakageduringnormaloperation.Thetemperatureofthewater Attachment1toAEP:NRC:1170DPage3inthepressurizerrelieftankismeasuredandindicatedbyinstrumentnumberNTA-351.Analarmresultingfromthissignalisactuatedbyhigh~atertemperature,andinformstheoperatorthatcoolingofthe"pressurizerrelieftankcontentsisrequired.Thepressurizerrelieftankpressuretransmitter(NPA-351)operatesahighandlowpressurealarm.Thepressurizerrelieftankleveltransmitter(NLA-351)providesasignalforalevelindicationandahighandlowtanklevelalarms.Consequently,adischargefromoneofthepressurizersafetyvalveswillcausetheseinstrumentstoprovideanalarmindicationinthecontrolroom.Alldownstreamtemperature,pressure,andtanklevelindicationsarenotenvironmentallyqualified.Inanemergencysituation,theoperatordoesnotuseasignalfromthepressurizersafetyvalvepositionindicatoracousticmonitorsas,anactioninitiator.Theoperatorrespondstootherindicationsoflossofreactorcoolantinventory.Astheoperatordoesnothavedirectcontrolofthepressurizersafetyvalve,"where"thelossofinventoryisoccurringisoflessimportancethancompensatingforthelossofinventory.Inaddition,theCookNuclearPlantemergencyoperatingproceduresdonotreferencethepressurizersafetyvalvepositionindicatoracousticmonitorsforthereasonstatedabove.ReasonforProosedTechnicalSecificationChaneReuestTherequesttoremovethefootnotesandreferencestothefootnotesismadeinordertoclarifytheTechnicalSpecificationsandtoremoveunnecessaryinformation.Withregardtochangingtheminimumnumberofoperablechannelsfromonepervalvetotwooutofthreetotal,inlettersAEP:NRC:1170datedDecember16,1991andAEP:NRC:1170BdatedJanuary22,1992,amendmentswererequestedtotheUnit2andUnit1TechnicalSpecifications,respectively,tointroducethe*****footnoteswhichareproposedtobedeletedbythisletter.Theserequestsweremadeonanexpeditedbasistoprecludeanunwarrantedshutdownofeachreactorunit.Webelievethereliefthatwasgrantedonatemporarybasiscanbesafelyimplementedonapermanentbasis.Sincemostofthepressurizersafetyvalvepositionindicatoracousticmonitorsystemisinaccessibleduringpoweroperation,thishastheadvantageofpotentiallyeliminatingtheneedforplantshutdownorforexpeditedoremergencyT/Sreliefshouldanacousticmonitorbecomeinoperableinthefuture.Webelievethathavinganacousticmonitorinoperabledoesnotwarrantreactorandplantshutdown.AstheT/Ssarecurrentlystated,shouldonepressurizersafetyvalvepositionindicator 4

Attachment1toAEP:NRC:1170DPage4acousticmonitorbecomeinoperable,itmustberestoredtooperablestatuswithinthirtydaysortheunitmustbeinhotshutdownwithinthesubsequenttwelvehours.Thermalcyclingfromunwarrantedplantshutdownsincreasesthelikelihoodofreactorvesselfatiguestresscyclesandunnecessarilychallengesthesafetysystems.Becauseasignalfromthepressurizersafetyvalvepositionindicatoracousticmonitorsisnotnecessarynorusedtoensurethesafeshutdownoftheunitevenifapressurizersafetyvalveisopenedorstuckopenduringanemergencysituation,webelievethataplantshutdownduetoaninoperableacousticmonitorwouldbeunwarranted.JustificationforProosedTechnicalSecificationChanesFortheremovalofthefootnotesandreferencestothefootnotes,theoperatingcyclesforwhichthefootnoteswerevalidhavebeenconcluded.Therefore,thefootnotesarenolongerapplicableandshouldberemoved.Forthereductionintheminimumnumberofchannelsoper'ablefromonepervalvetotwooutofthreetotal,first,thetemperaturesensorinstalleddownstreamofeachpressurizersafetyvalveprovidesindicationandalarmonboththecontrolroompanelandtheplantcomputer.Pastexperiencehasshownthesetemperaturesensorstobeeffectiveleakdetectiondevices.Inaddition,thepressurizersafetyvalvepositionindicatoracousticmonitoringsystemhasshowninthepastthatwhenoneofthepressurizersafetyvalvesopens,all-threesafetyvalvepositionindicatoracousticmonitorchannelsareactuated.Thisiscausedbythesensitivityoftheaccelerometersandthefactthatthethreesafetyvalvesandtheirassociatedacousticmonitorsensorsareincloseproximitytoeachotherandtiedtogetherthroughcommonpiping.Thus,ifthepressurizersafetyvalveassociatedwiththeinoperablepositionindicatoracousticmonitorchanneldischarges,theremainingtwopositionindicatoracousticmonitorchannelswouldalerttheoperator,Anadditionalbenefittothiseffectisthatitalsoenablestheoperatortodiscriminateavalidalarmindicationfromaninvalidalarmindication.Otherinstrumentationexiststhatprovidestheoperatorwithindicationofpressurizersafetyvalveactuation.Thepressurizersafetyvalvesdischargeintothepressurizerrelieftank.Thetemperature,pressure,andliquidlevelofthetankareindicatedandalarmedinthecontrolroom.Achangeintheseparameterswouldalarmandalerttheoperatorofasafetyvalvedischarge.condition.

Attachment1toAEP:NRC:1170DPage5Finally,inanemergencysituation,theoperatordoesnotuseasignalfromthepressurizersafetyvalvepositionindicatoracousticmonitorsasanactioninitiator.Theoperatorrespondstootherindicationsoflossofreactorcoolantinventory.As'theoperatordoesnothavedirectcontrolofthepressurizersafetyvalve,"where"thelossofinventoryisoccurringisoflessimportancethancompensatingforthelossofinventory.Inaddition,theCookNuclearPlantemergencyoperatingproceduresdonotreferencethepressurizersafetyvalvepositionindicatoracousticmonitors.5.0NOSIGNIFICANTHAZARDSDETERMINATIONWehaveevaluatedtheproposedT/Ssexemptionandhavedeterminedthatitshouldnotrequireasignificanthazardsconsiderationbasedonthecriteriaestablishedin10CFR50.92(c).OperationoftheCookNuclearPlantinaccordancewiththeproposedamendmentwillnot:(1)Involveasinificantincreaseintherobabilitorconseuencesofanaccidentreviouslevaluated.Althoughtheproposedexemptionresultsintheoperatorhavingonelesssourceofinformationonplantstatus,itdoesnotcreateasignificantincreaseintheprobabilityorconsequencesofanaccidentpreviouslyevaluated.Theacousticmonitorsdonotperformafunctionvitaltosafeshutdownortotheisolationofthereactor,orthereactorcoolantsystempressureboundary,noristhereamechanisminvolvinganoperableorinoperablepressurizersafetyvalveacousticmonitorwhichwouldinitiateanaccident.ThesemonitorswereaddedtomeettherequirementsofNUREG-0578andNUREG-0737.Duringnormaloperations,otherinstrumentationexiststhatprovidestheoperatorwithindicationofsafetyvalveactuation.Theacousticmonitorsarenotnecessarytoandarenotusedintheemergencyoperatingprocedures.Inaddition,theacousticmonitorsbeinginoperablewillnotresultinanuncontrolledreleaseofradiationtotheenvironmentandwillnotinitiateanaccident.Finally,althoughtheoperatormayhaveonelesschanneloperable,theoperatorreceivesnolessinformationthanifallthreechannelsareoperablebecause"onevalveopeningcausesalloperablechannelstoactuateTherefore,weconcludethattheproposedT/Sschangesdonotinvolveasignificantincreaseintheprobabilityorconsequencesofanaccidentpreviouslyevaluated.(2)Createtheossibilitofanewordifferentkindofaccidentfromanreviouslanalzed.Aspreviouslystated,thepurposeoftheacousticmonitoris'toprovidetheoperatorwithinformationregardingsafetyvalveposition EtCII' Attachment1toAEP:NRC:1170DPage6thatmayassistinthemitigationoftheconsequencesofanaccident.Specifically,itprovidesinformationthatasafetyvalvehaslifted.However,theoperatorhasothermechanismsforobtainingequivalentinformation.Inaddition,thesignalsgeneratedbyanacousticmonitordonotinitiateanyotherequipmentactuation,norwilltheinoperabilityofanacousticmonitorinitiateanyaccident.Consequently,theproposedT/Sschangesdonotcreatethepossibilityofanewordifferentkindofaccidentfromanypreviouslyanalyzed.(3)InvolveasinificantreductioninamarinofsafetTheproposedT/Sschangesresultintheoperatorpotentiallyhavingonelesssourceofinformationonplantstatus.However,webelievethemarginofsafetyisnotreducedforseveralreasons.First,theoperatorisprovidedwithotherviableflowdetectiondevicestodeterminepressurizersafetyvalveposition,i.e.,thetemperaturesensoronthedischargelineassociatedwiththeinoperableacousticmonitor,andpressurizerrelieftanklevel(NLA-351),temperature(NTA-351)andpressure(NPA-351)indications.Also,theacousticmonitorsarenotusedbytheoperatorsinanemergencysituation,as.theoperatorreliesonotherindicationsoflossofreactorcoolantinventorypertheemergencyoperatingprocedures.Inaddition,previousexperiencewithth'epressurizersafetyvalvepositionindicatoracousticmonitoringsystemhasshownthat,whenanyoneofthepressurizersafetyvalvesopens,allthreesafetyvalvepositionindicatoracousticmonitorsareactuated.Becauseofthis,theoperatorreceivesnolessinformationregardlessifonlytwoorthreechannelsareoperable.Basedontheabove,webelievethathavinganacousticmonitorinoperabledoesnotwarrantreactorandplantshutdown.AstheT/Ssarecurrentlystated,shouldonepressurizersafetyvalvepositionindicatoracousticmonitorbecomeinoperable,itmustberestoredtooperablestatuswithinthirtydaysortheunitmustbeinhotshutdownwithinthesubsequenttwelvehours.Thermalcyclingfromunwarrantedplantshutdownsincreasesthelikelihoodofreactorvesselembrittlementandunnecessarilychallengesthesafetysystems.Becauseasignalfromthepressurizersafetyvalvepositionindicatoracousticmonitorsisnotnecessarynorusedtoensurethesafeshutdownoftheunitevenifapressurizersafetyvalveisopenedorstuckopenduringanemergencysituation,webelievethataplantshutdownduetoaninoperableacousticmonitorwouldbeunwarranted.WebelievethattheunitcanbeoperatedsafelyandthatwewouldstillmeettheintentofNUREG-0538andNUREG-0737withonlytwooutofthreepressurizersafetyvalvepositionindicatoracousticmonitorsoperable.
