Title | Licensee-Identified Violation |
Description | The licensee identified a finding of very low safety significance (Green) and an associated NCV o f TS 5.7.2, which requires, in part, that each entryway into High Radiation Areas ( HRAs) with dose rates greater than 1.0 rem/hour at 30 centimeters from the radiation source or any surface penetrated by the radiation, but less than 500 rads/hour at 1 meter from the radiation source or from any surface penetrated by the radiation source shall be provided with a locked or continuously guarded door or gate that prevents unauthorized entry. Contrary to the above, on May 4, 2017, the licensee failed to lock or continuously guard an entryway into a HRA with dose rates greater than 1.0 rem/hour at 30 centimeters from the radiation source or any surface penetrated by the radiation, but less than 500 rads/hour at 1 meter from the radiation source or from any surface penetrated by the radiation source. Specifically, an entryway was left unguarded when the individual assigned to guard the entryway left the area prior to another guard being stationed. This issue was identified by a radiation protection technician who immediately stationed another guard. This issue was entered into the licensees CAP as CR PL 2017 02160. The failure to continuously guard the HRA entryway was a performance deficiency that was within the licensees ability to foresee and should have been prevented. The performance deficiency was more than minor because it was associated with the Program and Process attribute of the Occupational Radiation Safety cornerstone and adversely affected the cornerstone objective of ensuring the adequate protect ion of worker health and safety from exposure to radiation. The finding was determined to be of very low safety significance (Green) because it did not involve as -low -as-reasonably -achievable planning or work controls, there was no overexposure or substantial potential for an overexposure, and the licensees ability to assess dose was not compromised. |
