| Description | Title 10 CFR 50.65(a)(4) states, in part, that before performing maintenance activities, the licensee shall assess and manage the increase in risk that may result from the proposed maintenance activities. Contrary to the above, on February 27, 2012, the high pressure core spray system was made unavailable during surveillance testing without performing a risk assessment prior to conducting testing. The documented the issue in the corrective action program as Action Request AR 258712. This violation is of very low safety significance because the risk deficit during the time of the surveillance was calculated to be less than 1.0E-6. |
