DOCKET NUMBER: 050-265 V.

EVENT DESCRIPTION

On June 23, 1980, whlie ' performing procedure QOS 2300-2, HPCI Pump Oper-ability Test, an oil leak was found on the discharge line of the Emergency Oil Pump which supplies lubrication to the inboard bearing of the main HPCI pump. The surveillance test was completed satisfactorily and a work request,was initiated to repair the leak.

PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The safety implications of this occurrence were minimal, because HPCI was demonstrated to be operable with the oil leak, and would have fulfilled its intended function had it been needed. All other redundant safety systems were operable during the leak, and during the repair.

CAUSE

The cause of the oil leak was apparent cracked threads on a nipple on the discharge line of the oil pump.

Vill.

CORRECTIVE ACTION

The HPCI Pump Operability Test was completed satisfactorily.

Unit Two HPCI was then taken out of service and declared inoperable. An orderly shutdown was initiated as per Technical Specification 3.5.C.3 Load was reduced at a - rate of 25 MWe/ hour for a period of thirty-five minutes at which time the leak repair was complete and the HPCI system was returned to service.

Repairs consisted of removing the defective threads on the nipple, recutting new threads and reinstalling the nipple.
