CAUSE DESCRIPTIOil AND CORRECTIVE ACTI0flS:

(C0llTItajED)

Although it was apparent that fire retardant material had been applied (after modifications took place) the quality of application was inadequate. A training program including maintenance and quality control personnel has recently been completed and all personnel were made aware of the importance of maintaining proper fire barriers throughout the plant. The training program is being incorporated 'nto a general orientation program to ensure all site (new and present) personnel are adequately trained. in the fire protection program.

Additionally, an inspection has been conducted to verify the status of all fire barrier penetrations in all fire walls protecting safety-related areas.

As a result of this inspection, 21 additional inadequately sealed penetrations ir. the cable spreading room, control room, control room ventilation room, and auxiliary feedwater pump room were identified and repaired.

It is believed that these penetrations were not properly sealed during construction.
