EVENT DESCRIPTION

On November 3,1980, Unit One was shutdown for a refueling and maintenance i

outage. A portion of the work involved extensive modification to the suppression chamber.

In order to make the weld fit-ups of the T quencher i

spargers, excessive mechanical movement of the safety relief valve dis-charge lines was required. Noting this, the line w.periencing the maxi-mum motion was analyzed for that motion.

Based on the results, a special examination was performed on the vent header penetrations using the magnetic particle method.

During this inspection, a crack was found on line 1-3019E-8", discharge from the 1-203-3E electromatic relief valve j

at the penetration. The crack was approximately 4 inches long, 1/8 to j

1/4 inch deep, and had a bright appearance.

VI.

PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The Auto-Depressurization system is not required to be operable with the unit shutdown. Therefore, this occurrence had minimal consequences. All other penetrations were examined and no indications were found.

j Vll. CAUSE:

Construction personnel used excessive force on the pipe causing failure in the area of highest stress. The penetration rigidly supports the pipe therefore, any force would concentrate near this point. On line 1-3019E-8" the force used to make a proper weld fit-up was large enough to crack the pipe.

CORRECTIVE ACTION

The crack will be repaired prior to the startup.. Therefore, the Auto-l Depressurization will be fully operable prior to startup. -The other safety relief valves have been inspected and no cracks were found. A 4

meeting was held with the contractor involved in which guidance on the allowable pipe movement limits to achieve proper fit-up was supplied.
