At 0400 while investigating the cause for dirty oil in the inboard turbine bearing on #11 AFW Pump, the mechanic discovered that the bearing journal had become worn to the point where replacement was required (T.S. 3.7.1.?). #11 AFW Pump was repaired and returned to service af ter successfully completing a post mainten nce operational test at 2005 on 02-19-81. #12 AFW Pump, which recently had its inboard turbine bearing replaced in January 1981, rernained operable.

CAUSE DESCRIPTION AND CORRECTIVE ACTIONS (CONT'D)

The bearing was scored, and babbit material was found suspended in the oil. T! as osmage could be caused by improper oil level or contaminated oil. Markings nave

^

been added to the sightglasses to maintain proper oil level and the Preventive Maintenance Program has been modified to increase the oil change interval.

Insulation has also been installed on the turbine casing near the bearings to prevent excessive heat transfer to the bearing surface.
