At 1440 on 11/01/85 with the plant in operational condition 2 (startup), a Division I Reactor Water Cleanup isolation occurred.

While the operator was taking RWCU room temperatures for Surveillance Test Procedure STP-000-001 " Daily Operating Logs,"the failed to place the RWCU isolation bypass switch in bypass.

When the area selection switch was actuated, the isolation occurred.

The isolation was reset and RWCU reinitiated.

There was no effect on the safe operation of the plant or the health and safety of the public, hhbD 0

At 1440 on 11/01/85 prior to taking Reactor Water Cleanup (RWCU)

System area temperatures for Surveillance Test Procedure STP-000-001

" Daily Operating Logs," the operator failed to place the RWCU isolation bypass switch in bypass.

When the first area selection switch was selected, an RWCU Division I isolation occurred.

The isolation was caused by a

combination of system design and operator error.

The system, as presently designed, must be in isolation bypass prior to checking area temperatures.

An operator aid was posted to alert the operators of this problem, but the operator failed to read it.

In an attempt to prevent recurrence the operator was reinstructed to

observe, read and follow the operator aids.

Secondly, a modification (JMR 85-0589, MWR 15553) to add surge suppressors to the-circuits to climinate the need to put the system in bypass is in progress.

Completion of this modification is expected by 2/1/86.

There was no effect on the safe operation of the plant or the health and safety of the public.

Additionally, continued operation of the plant in this condition will not affect safe operation of the plant and will have no effect on the health and safety of the public, because these valves actuate (close) in a safe direction.
