PRELIMINARY NOTIFICATION June 30, 2021 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE - PNO-III-21-001 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. Some of the information may not yet be fully verified or evaluated and is basically all that is known by the Region III staff on this date.

SUBJECT:

UNPLANNED SHUTDOWN GREATER THAN 7 DAYS DUE TO MAIN STEAM LEAKAGE On June 22, 2021, at 11:31 p.m. EDT, with the plant at full power, operators at the Donald C. Cook Nuclear Plant manually tripped the Unit 2 reactor in response to a steam leak in the turbine building. The steam leak was quickly isolated and decay heat was initially removed by the steam dump system. No personnel were injured during the leak.

On June 24, 2021, the licensee began removing decay heat using the steam generator powered operated relief valves to facilitate personnel access during repair/investigation activities. There was no radioactive release.

All other systems functioned normally. Unit 1 remains at full power. There was no impact on public health.

Region III received initial notification of this occurrence by telephone call from the licensee at 12:25 a.m. EDT on June 23, 2021. The resident inspector responded to the plant, walked down the area of the steam leak and monitored plant parameters.

The State of Michigan has been notified.
