SUMMARY

OF MEETING WITH UNION ELECTRIC COMPANY REGARDING STUCK.

REACTOR' VESSEL HEAD CLOSURE BOLTS On October 23, 1987, the staff met with Union Electric Company regarding.the  !

stuck reactor vessel head closure bolts at Callaway. . Meeting attendees are )

listed in Enclosure 1. Slides used during the presentations are in Enciosure 2. 1 At the staff's request, the licensee presented the sequence of events',. root; cause, corrective action, action to prevent ~ recurrence, and current plans.-

Included in the presentation were discussions on.the licensee'.s~ technical .

specification interpretation, 10 CFR 50.59 review and safety evaluation for Cycle 3 operation.

The staff agreed to allow the licensee to. enter modes 5, 4 and 3,. but. requested that the licensee formally submit their. safety analysis-for staff review and approval before. proceeding to criticality. In addition to the analysis regarding'.-

one closure bolt not fully tensioned, the staff requested an ~ analysis for three-adjacent closure bolts not fully tensioned. Additionally, the' staff requested that the licensee commit to: (1) a strict leakage limit on the: vessel flanger (i gpm or lower);. (2) report the results of the tensioning; and.(3) remove all>

stuck bolts at the next refueling. outage.-
