Title | Failure to Update Emergency Operating Procedures |
Description | The inspectors reviewed a self-revealing non-cited violation of Technical Specification 5.4.1(b) for failure to maintain emergency operating procedures after personnel reviewing a temporary modification failed to identify and change affected emergency operating procedures. Specifically, emergency operating procedure EOP E-0.1, Reactor Trip Response, Revision 28, was not updated to be consistent with a temporary modification of steam generator water level low-low bistable setpoints. The licensee entered the condition into the corrective action program as Notifications 50517883, 50520697, and 50518355. The failure to update emergency operating procedure E-0.1 Reactor Trip Response, Revision 28, to account for higher low-low water level bistable reset setpoints introduced by Temporary Modification 60044709 was a performance deficiency. The finding was more than minor because it was associated with the procedure quality attribute of the Initiating Events cornerstone. Using Inspection Manual Chapter 0609.04, Appendix A, Exhibit 1, Initiating Events Screening Questions, this finding was determined to be of very low safety significance (Green) because the finding does not represent a loss of system and/or function and does not represent an actual loss of function of at least a single train for greater than its Technical Specification allowed outage time, or two separate safety systems out-of-service for greater than its Technical Specification allowed outage time. This finding had a crosscutting aspect in the area of human performance, associated with the resources component, because the licensee did not ensure complete, accurate, and up-to-date procedures were available and adequate to ensure nuclear safety. |
