4 ATTACHMENT 2 MAIN STEAM SAFETY VALVE AND SAFETY RELIEF VALVE FAILURES AND CHALLENGES (Ref.: NUREG-0737, Action Item II.K.3.3)

During 1998, there were no operational failures or challenges to the Safety Valves and Safety Relief Valves.

As corrective action for a known issue with bonding of pilot assembly discs, Cooper Nuclear Station executed a mid-cycle outage in March 1998 to replace all eight Safety Relief Valve pilot assemblies I with modified pilot assemblies utilizing Stellite 21 pilot discs. Five of the pilot assemblies removed during the mid-cycle outage subsequently failed as-found testing requirements. In addition, one  ;

removed pilot assembly was damaged and could not be tested. The corrective action taken during '

the mid-cycle outage to install the modified pilot assemblies was designed to resolve the known pilot ,

assembly disc to seat bonding issue and limit future failures. In addition, Improved Technical  !

Specifications subsequently increased the setpoint tolerance from *1% to 3% which provides a more realistic acceptance criteria for valve testing.

Two complete Safety Relief Valves and six Safety Relief Valve pilot assemblies were replaced during Refueling Outage 18. All Safety Relief Valves that were removed during Refueling Outage 18 are i scheduled for as-found testing in late April 1999. Testing results will be reported in a subsequent Monthly Operating Report.

One Safety Valve was removed during Refueling Outage 18, satisfactorily tested, and reinstalled.
