Reportable -. Occurrence Report No. 321/1980-23.

DuringEreactor startup with' the mode switch in. startup' and hot standby position,

the IIPCI System did nor -becomo operational: at 113 psig 'as specified in Technical

- (Specifications Section 3.5.D.'1.

There were no effects upon public health and

~

r safety'duetto.this event. This is a repetit!1ve occurrence as last _ reported on Reportabic Oclcurrene,c - Report. No. 321/1979 106. This report addressed i

the problem of IIPCI Lisointing at 116' psig. Tachnical Specification Section 3.5-D.1 requires that 11PCI be operational:if.the reactor pressure is greater thanill3Lpsig. Prior 'to LER.50-321/1979-106, the switches, IE41-N001A-D, setpoint was:125 poig. '-This setpoint complied with Tech Specs requirement Table 3.2.2 which requires the switches to be set equal to or greater than 1000psigi sbut ;does not comply ~ with Tech Specs Section. 3.5-D.1 which required.

_'llPCI tolhe. operational rt 113 psig. The inherent: prob 1cm was first recognized at.'this point. A' design change-was submitted to change the netpoint to a'value 6

greaterithan'100 psig but Icos than 113 psig.

e j.q

.On February 24,1980' 11PCI'did not become operational at reactor pressure c qual to 'or; greater. than ' 113 psig. The calibration of the llPCI Steam Line

({,,,

c Low Pressure Switches was checked and found as follows:-

N001A. trip 90.7' psi, reset 127.7 psi.

~ N001B trip 98.7 psi,. reset.123.7 pai.

N001C trip 100.7 psi, reset.13'2.7 psi.
N001D.LYip 98.7 pai, reset 123.7 psi.

1Three of the-switches A,' B and D were found below the acceptance criteria

~

K

~ of' actuation ~at - greater thmi 100 psig.: The exoccted actuation point is

'The dif ference' between the ' expected actuation and the L'

- 10. 5 - c5. psi.

6 Jactual trip point is attributed to instrument setpoint drift.

i 1The manufacturer of these switches is Darksdale.. The' specifications of-the

('

switch permit-a react span different'.alc of 11 to 40 psig and.is not adjustabic.

Decause of the wide differential, the switches cannot be adjusted to meet the Spresent Tech Spec requirements.-

The planned eorrective;" action willIinclude t!io following.
