GE had recognized that the Clinton local panels were conditionally released for manu-facture and conditionally released for shipment due to lack of qualification. Nevertheless, for eight of the local panels de-livered, the documentation for product quality certification (PQCs) did not acknowledge this conditionality and thus reflect a breakdown in the quality assurance system.
