This condition resulted in an inoperable train of the containment spray system for an entire operating cycle and also provided a reactor coolant system diversion flow path if shutdown cooling was initiated following certain postulated accident conditions.A Notice of Violation is included based on a violation of 10 CFR Part 50, Appendix B, Criterion V, "Instructions, Procedures, and Drawings," for conducting maintenance activities without procedures that were appropriate to the circumstances. Specifically, the maintenance and post-maintenance procedures did not require actions to verify the correct orientation of the valve. As a result, the valve was installed in the wrong orientation during maintenance, and post-maintenance testing did not detect the improper reassembly prior to returning the valve to service. PRELIMINARY INFORMATION - NOT FOR PUBLIC DISCLOSURE UNTIL VERIFICATION THAT LICENSEE HAS RECEIVED ACTION May 23, 2007 EN-07-036 OFFICE OF ENFORCEMENT NOTIFICATION OF SIGNIFICANT ENFORCEMENT ACTION Licensee: Omaha Public Power District (EA-07-047)

Fort Calhoun Station Docket No. 05000285

Subject:

ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violation associated with a White significance determination process finding will be issued on or around May 29, 2007, to Omaha Public Power District as a result of inspections at the Fort Calhoun Station. This White finding, an issue of low to moderate safety significance which may require additional U.S. Nuclear Regulatory Commission inspection, involved the improper installation of the valve disk of a Containment Spray Header Isolation Valve. The improper installation resulted in a condition in which the actual position of the valve was nearly opposite of the indicated position.

This condition resulted in an inoperable train of the containment spray system for an entire operating cycle and also provided a reactor coolant system diversion flow path if shutdown cooling was initiated following certain postulated accident conditions.

A Notice of Violation is included based on a violation of 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, for conducting maintenance activities without procedures that were appropriate to the circumstances. Specifically, the maintenance and post-maintenance procedures did not require actions to verify the correct orientation of the valve. As a result, the valve was installed in the wrong orientation during maintenance, and post-maintenance testing did not detect the improper reassembly prior to returning the valve to service.

It should be noted that the licensee has not been specifically informed of the enforcement action. The schedule of issuance and notification is:

Mailing of Final Significance Determination May 29, 2007 Telephone Notification of Licensee May 29, 2007 The State of Nebraska will be notified.
