July 12, 2011 EN 11-021 OFFICE OF ENFORCEMENT NOTIFICATION OF SIGNIFICANT ENFORCEMENT ACTION Licensee: Omaha Public Power District EA-11-025 Fort Calhoun Station Docket No. 50-285

Subject:

ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission of an escalated enforcement action that will be issued on or about July 15, 2011, to Omaha Public Power District (Licensee) as a result of inspections at Fort Calhoun Station. The enforcement action consists of a White finding associated with a violation of 10 CFR 50, Appendix B, Criterion XVI, Corrective Action.

The White finding, an issue with low to moderate safety significance, along with a violation, involved the licensees failure to assure that the cause of a significant condition adverse to quality was determined and corrective actions taken to preclude repetition. Specifically, on November 3, 2008, the licensee determined that the shading coil in the reactor trip contactor (M2) had fallen out of its recess and became loose material in the contactor. Although the licensee determined that the trip contactor was obsolete and needed replacement, the licensee manually pressed the shading coil back into place and continued operations. On March 25, 2010, the licensee again identified the shading coil had fallen out, as evidenced by chattering noise, and on March 31, 2010, the licensee pressed the shading coil back into place. Due to a lack of replacement parts, the licensee determined the contactor would be left as is and they would continue to operate.

The licensee did not identify that this issue could impact the operation of the contactor, beyond the presence of a chattering noise. On June 14, 2011, the licensee performed a quarterly surveillance test on the reactor trip contactors in the reactor protection system. During this test, the M2 contactor failed to open as required. The licensee subsequently determined that the cause was due to a shading coil falling out of it recess, breaking apart, and lodging in the contactor mechanism such that it bound its contacts in the closed position. The licensee failed to identify that the loose parts in the trip contactor represented a potential failure to preclude repetition of this significant condition adverse to quality that subsequently resulted in the contactor failing, contrary to 10 CFR 50, Appendix B, Criterion XVI.

It should be noted that the licensee has not been specifically informed of the enforcement action.
