The NRC examiners conducted operator licensing initial examinations in accordance with the | |||
guidance in NUREG-1021, Revision 9, Operator Licensing Examination Standards for Power | |||
Reactors. This examination implemented the operator licensing requirements of 10 CFR | |||
§55.41, §55.43, and §55.45. | |||
The NRC administered the operating tests during the period of March 17-27, 2008. Members of | |||
the Surry Power Station training staff administered the written examination on April 9, 2008. | |||
The written examinations were developed by the NRC and the operating tests were developed | |||
by the Surry Power Station training staff. | |||
Four Senior Reactor Operator (SRO) applicants and eight Reactor Operator (RO) applicants | |||
passed both the written and operating examinations. One RO applicant failed the written | |||
examination. Four SRO and eight RO applicants were issued operating licenses. | |||

a. Inspection Scope | |||
The Surry Power Station training staff developed the operating test. NRC regional | |||
examiners reviewed the proposed examination material to determine whether it was | |||
developed in accordance with NUREG-1021, Operator Licensing Examination | |||
Standards for Power Reactors, Revision 9. Examination changes agreed upon | |||
between the NRC and the licensee were made according to NUREG-1021 and | |||
incorporated into the final version of the examination materials. | |||
The examiners reviewed the licensees examination security measures while preparing | |||
and administering the examinations to ensure examination security and integrity | |||
complied with 10 CFR 55.49, Integrity of Examinations and Tests. | |||
The examiners evaluated four SRO applicants and nine RO applicants who were being | |||
assessed under the guidelines specified in NUREG-1021. The examiners administered | |||
the operating tests during the period of March 17-27, 2008. Members of the Surry | |||
Power Station training staff administered the written examination on April 9, 2008. The | |||
evaluations of the applicants and review of documentation were performed to determine | |||
if the applicants, who applied for licenses to operate the Surry Power Station, met | |||
requirements specified in 10 CFR Part 55, Operators Licenses. | |||

reactor failed to trip is required to meet the minimum requirements for this task (i.e. this is the | |||
minimum required for a satisfactory Performance Indicator). This evaluation criterion was | |||
subsequently identified in DNAP-2605 (Emergency Preparedness Performance Indicators). | |||
Attachment 2 of this procedure only requires a description of the event, which is meant to | |||
indicate the reason for entering the emergency plan. As such, the critical task should state, at a | |||
minimum, The reactor failed to trip. | |||
Recommend replacing the critical task for item #5 to read as follows: The reactor failed to trip | |||
(or words to this effect). | |||

4OA5 Operator Licensing Initial Examinations
a. Inspection Scope
The Surry Power Station training staff developed the operating test. NRC regional
examiners reviewed the proposed examination material to determine whether it was
developed in accordance with NUREG-1021, Operator Licensing Examination
Standards for Power Reactors, Revision 9. Examination changes agreed upon
between the NRC and the licensee were made according to NUREG-1021 and
incorporated into the final version of the examination materials.
The examiners reviewed the licensees examination security measures while preparing
and administering the examinations to ensure examination security and integrity
complied with 10 CFR 55.49, Integrity of Examinations and Tests.
The examiners evaluated four SRO applicants and nine RO applicants who were being
assessed under the guidelines specified in NUREG-1021. The examiners administered
the operating tests during the period of March 17-27, 2008. Members of the Surry
Power Station training staff administered the written examination on April 9, 2008. The
evaluations of the applicants and review of documentation were performed to determine
if the applicants, who applied for licenses to operate the Surry Power Station, met
requirements specified in 10 CFR Part 55, Operators Licenses.
b. Findings
The NRC determined that the details provided by the licensee for the walkthrough and
simulator tests were within the range of acceptability expected for a proposed
examination.
Four SRO applicants and eight RO applicants passed both the operating test and written
examination. One RO applicant failed the written examination.
The final RO and SRO written examinations with knowledge and abilities (K/As) question
references/answers, examination references, and licensees post examination
comments may be accessed in the ADAMS system (ADAMS Accession Numbers,

Copies of all individual examination reports were sent to the facility Training Manager for
evaluation and determination of appropriate remedial training.

Facility Comment
The number in the answer key for the N-44 Upper Tilt Calculation was listed as .9895. The
correct answer should have been .9858. This was not a critical task in this JPM.
Recommend replacing .9895 with .9858 in the answer key for the N-44 Upper Tilt Calculation.
NRC Resolution
Recommendation accepted. The facility comment corrects an error in the answer key.

Facility Comment
Item 5 of the transmittal form had the critical task listed as Unit 1 reactor failed to trip. The
reactor is now tripped (or words to this effect).
After conferring with the Emergency Preparedness group, only notifying the State that the
reactor failed to trip is required to meet the minimum requirements for this task (i.e. this is the
minimum required for a satisfactory Performance Indicator). This evaluation criterion was
subsequently identified in DNAP-2605 (Emergency Preparedness Performance Indicators).
Attachment 2 of this procedure only requires a description of the event, which is meant to
indicate the reason for entering the emergency plan. As such, the critical task should state, at a
minimum, The reactor failed to trip.
Recommend replacing the critical task for item #5 to read as follows: The reactor failed to trip
(or words to this effect).
NRC Resolution
Recommendation accepted. Per DNAP-2605, Attachment 2, the description of the event (Item
5 of the transmittal form) is adequate when the description of the event sufficiently describes the
plant conditions that caused the facility to enter into the classification that is being reported.
Additionally, a description of the event that is sufficient to explain the plant conditions that
caused the facility to enter the event classification being reported is sufficient to accomplish the
JPM task consistent with the initiating cues provided to the applicants.

or inspection findings and, without further verification and review in accordance with IP 71111.11, are not indicative of noncompliance with 10 CFR 55.46. No licensee action is
required in response to these observations.
During performance of a simulator JPM the examiner noticed that 1-SI-FI-1932, HHSI Flow to
Hot Leg Loop C meter, was reading low. The facility generated Simulator Modification Record #
200803251315 to document the condition and subsequently replaced the meter.
