On November 11, 1985, Illinois Power Licensing and Safety Department received a 10CFR50.55(e)/Part 21 referral from
- Illinois Power Nuclear Station Engineering Department concerning 410 stainless steel valve stem corrosion failures. Based on an evaluation of this matter, Illinois Power Company is providing the following information to the Commission in accordance with the requirements of 10CFR Part 21.21(b)(3).
(i) D. P. Hall, Vice Presi(ent of Illinois Power Company, by means of this report.
(ii) The basic components involved in this reportable defect are valve stems and shafts manufactured from Type 410 Stainless Steel (S.S.) that were improperly heat treated.
(iii) The valve stems and shafts are supplied by the following valve manufacturers:

r U 600590 Ll6-86(05- 28)-L 1A.120 (iv) .The nature of the defect involves stress corrosion failures of type 410 S.S. valve stems and shafts. This defect is associated with excessive hardness (in excess of 30 Rockwell C) caused by improper heat treatment.
Although type 410 S.S. is defined as a stainless steel because of its alloy content, it is also a high chromium, very hardenable steel. Cooling this material in air from the 1900 to 1700*F temperature range results in a surface hardness of up to HRC45 and high internal stresses. This practice results in low and erratic impact properties and poor resistance to stress corrosion which can result in cracking which occurs in areas where there could be a concentration of corroding chemicals, such as at gland packing.
' One hundred and sixty-six valves were identified (see Attachment 1) as active valves with type 410 stem or shaft material. Upon inspection, fourteen (14) of these valves were determined to have "hard stems" (in excess of a 30 Rockwell C). The failure of these thirteen valves could render the High Pressure Core Spray, Low Pressure Core Spray, Reactor Core Isolation Cooling, Shutdown Service Water, RHR "C" Loop and Reactor Feedwater Systems inoperable.
Failure of the above systems would impact operability of all the Emergency Core Cooling Systems.

Furthermore, the failure of lE22-F010 and F011 could violate the Off Site Dose Release Rate (10CFR100 Limits). This violation could occur if a LOCA required the HP system to draw from the suppression pool and the 1E22-F010/F011 valves had failed-open, permitting the radioactive suppression pool water to be released to the RCIC tank.
Based on the above review, this defect could have resulted in a condition which could have created a substantial safety hazard and is therefore evaluated to be reportable under the provisions of 10CFR21.
(v) July 17, 1985, I.E. Information Notice 85-59.
(vi) The quantity and location of the valves identified as having type 410 S.S. stems with excessive hardness are listed in Attachment No. 2.
(vii) One hundred and sixty-five (165) of the one hundred and sixty-six (166) safety related active valves with type 410 S.S. stems or shafts identified in Attachment #1 were tested for stem hardness. This testing identified fourteen (14) valves with "hard stems" (in excess of a 30 Rockwell C). The fourteen (14) "hard stems" were
- Ll6-86( 05-28 )-L 1A.120 ultrasonically tested (UT) or liquid penetrant (LP) tested to determine if the stems were cracked. Based on the results of these tests no cracked stems were identified. Although no cracked stems were identified, Illinois Power Company will replace the. fourteen (14)
"hard stems" during the first scheduled outage after replacement stems have been received. One (1) valve stem (1B21-F032A) remains to be tested. Testing of this stem will be completed by June 15, 1986. This stem will also be replaced at the first scheduled outage if testing shows that the hardness is in excess of a 30 Rockwell C. UT or LP testing will also be performed on the stem if the hardness is in excess of 30 Rockwell C.
(viii) General Electric has provided information concerning the effects of Intergranular Stress Corrosion Cracking (IGSCC) on type 410 S.S. This information was provided in General Electric's Valve Failure Analysis Letter CSI-625-85.
