Q2R16 was the only outage of the third period of the third inspection interval. The third period is scheduled to end on March 9, 2003. This report has been submitted to you in accordance with the requirements of ASME Section XI, Article IWA-6200.

Outage, Period and Interval Start and End Dates This Inservice Inspection Summary Report addresses examinations performed between the end of the previous refueling outage February 11, 2000 through the end of the current refueling outage. This current refueling outage was the 16 th refueling outage for Quad Cities Generating Station Unit Two. The outage started February 12, 2002 and ended March 5, 2002.

This refueling outage was the only outage during the third inspection period of the third inspection interval. The third inspection period of the third inspection interval is scheduled to end on March 9, 2003.

Tests, examinations, repairs and/or replacements associated with ASME Section XI were witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors. Non-ASME Section XI or augmented examinations were not witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors and are marked "Not Required" or "N/A" on the applicable examination reports. The Authorized Nuclear Inservice Inspectors are inspectors of the Hartford Steam Boiler Inspection

& Insurance Company of Connecticut (HSB-CT). The HSB-CT Regional Office is located at 2443 Warrenville Road, Lisle, Illinois, 60532-9871.

Examination Program Requirements The examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Division 1, "Rules for Inservice Inspection of Nuclear Power Plant Components", 1989 Edition, no addenda were implemented for Class 1, 2 and 3 components except where written relief has been granted by the NRC. Written Relief, in the form of Relief Request CR-33 approving Risk Inservice Inspection (RISI) as an inspection program alternative for Code Categories B-F, B-J, C-F-1 and C-F-2, was utilized during Q2R16. The ultrasonic examination areas for RISI components were increased, from the Section XI requirements, to meet the RISI Program.

The ultrasonic examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Appendix VII, 1995 Edition with Addenda through 1996, as required by 10 CFR 50.55a and amended by the Federal Register Notice 64 FR 51370 dated September 22, 1999 is executed through the Performance Demonstration Initiative (PDI) Program according to the schedule defined in 10 CFR 50.55a(g)(6)(ii)(C), for wrought austenitic piping welds, ferritic piping welds, bolts/studs, reactor vessel welds and full structural overlaid wrought austenitic piping welds.

The examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Division 1, "Rules for Inservice Inspection of Nuclear Power Plant Components, "1992 Edition, with 1992 Addenda were implemented for Class MC components except where written relief has been granted by the NRC.

3.1.Section XI Examinations Summary A combination of ultrasonic (UT), liquid penetrant (PT), visual (VT) and magnetic particle (MT) testing was performed prior to and during the 16 th refueling outage on ISI Program components. Examinations were performed by General Electric (GE) and Exelon Nuclear personnel. ISI inspections comprised of 69 volumetric (35 were RISI, Category R-A welds), 16 surface examinations and 50 visual examinations were performed by Exelon and GE certified personnel on Class 1, 2 and RISI components.

Volumetric, surface and visual examinations listed in Table 3.1-1 that did not meet coverage requirements of Section XI of the ASME Code coverage of greater than 90 percent of the specified examination volume and area, are listed in Table 3.1-1 A.

Supplemental examinations were performed on these components where practical to increase examination coverage. Relief requests will be generated and submitted at the end of the inspection interval as required by 10 CFR 50.55a(g)5(iii) and 50.55a(g)5(iv).

Volumetric and surface examinations were compared with the recorded results of previous examinations for all applicable Class 1, 2 and RISI components.

Pressure testing VT-2 exams not related to repair/replacement activities are documented in Table 3.1-2.

Exelon personnel performed functional tests on a total of thirteen snubbers for operability per the Technical Requirements Manual 10% sampling, Service Life Monitoring /Failure Trending Sample and ASME Section XI, Relief Request CR-09.

Eight (8) of the thirteen (13) snubbers were section XI. All snubbers were found acceptable for continued operation. Snubber testing activities are summarized in Table 3.1-3.

Table 3.1-4 summarizes the conditions noted and measures taken to correct the conditions.

3.2. Generic Letter 88-01 Augmented examinations required by Generic Letter 88-01, "NRC Position on Intergranular Stress Corrosion Cracking (IGSCC) in BWR Austentic Stainless Steel Piping", were performed as listed in Table 3.2-1. BWRVIP-75 "BWR Vessel And Internals Project, Technical Basis For Revisions To Generic Letter 88-01 Inspection Schedules" (as modified by associated NRC Safety Evaluation dated September 15, 2000), was implemented this outage. The original scope of the outage included 7 Category "C", 4 Category "D" and 9 Category "E" welds. MSIP was performed on all 4 of the Category "D" welds and they will be re-categorized to Category "C". Flaws were recorded on weld 02BD-F9 (Category "C") and resulted in a scope expansion of 7 of 20

No other flaws were identified within the expanded weld population. Weld 02BD-F9 will be monitored in accordance with GL 88-O1/BWRVIP-75. Engineering evaluation per IWB-3600 requirements determined the weld is acceptable for continued service.

During Q2R17, the weld will be addressed in accordance with the GL 88 01/BWRVIP-75 program.

3.3.

Invessel Inspections (IVI)

To implement the requirements of the Reactor Vessel Internals Program, General Electric (GE) was contracted to perform the invessel inspections. The following components and assemblies were examined; Jet Pump assemblies, Core Spray Internal Piping, Core Shroud, Vertical Welds, RPV Attachment Welds, Top Guide, Feedwater Spargers, Control Rod Guide Tubes and Vessel Interior. The only recordable indications were associated with Jet Pumps 1 and 7.

During Q2R16, in response to the Jet Pump Beam failure in Quad Cities Unit 1, all remaining BWR-3 style jet pump beams (18 total) in Quad Cities Unit 2, were replaced with BWR-4 style HTA beams. The Jet Pump (JP) assemblies inspections were performed in support of the beam replacements. During these inspections, a missing set screw at JP 17, and a gap at a set screw on JP 1 was identified. To correct these conditions, auxiliary wedges were fabricated and installed to restore the 3-point contact at the restrainers.

Corrective actions included:

"" Engineering evaluation was performed (as permitted by Relief Request "B" Recirculation Pump Casing PR-12) on the 2A MSIV Outboard valve and determined the leakage to be acceptable.

CRI~s 22-39, 22-23, 46-27, 38-0 Engineering evaluation was performed (as permitted by Relief Request 35 and 18-27 PR-12) on the 2B Recirculation Pump and determined the leakage to be acceptable.

Rods were leaking 300 drops/minute (DPM), 80 DPM, 120 DPM, 30 DPM and 60 DPM. Rods were scrammed per procedure QCOS 0201-08 and all leakage stopped.

Reference VT report # 022V2367 dated 03/01/02 InVessel Inspections Issues were identified with Jet Pump setscrews (one with gap and one with setscrew missing). Auxiliary wedges were installed to restore the required 3-point contact at Jet Pumps # 1 & # 17 the restrainers. These exams are not required by BWRVIP-41"BWR Jet Pump Assembly Inspection and Flaw Evaluation Guidelines" and were performed in support of the Jet Pump Beam Replacement Project.
