ANNUAL ASSESSMENT LETTER FOR QUAD CITIES NUCLEAR POWER STATION, UNITS 1 AND 2 (REPORT 05000254/2024006 AND 05000265/2024006)
Dear David Rhoades:
The NRC has completed its end-of-cycle performance assessment of Quad Cities Nuclear Power Station, Units 1 and 2, reviewing performance indicators (PIs), inspection results, and enforcement actions from January 1, 2024, through December 31, 2024. This letter informs you of the NRCs assessment of your facility during this period and its plans for future inspections at your facility. The NRC concluded that overall performance at your facility preserved public health and safety. The baseline inspection program was completed at your facility as defined in Inspection Manual Chapter 2515, Light-Water Reactor Inspection Program - Operations Phase.
The NRC determined the performance at Quad Cities Nuclear Power Station, Units 1 and 2, during the most recent quarter was within the Licensee Response Column, the highest performance category of the NRCs Reactor Oversight Process (ROP) Action Matrix, because all inspection findings had very low safety significance (i.e., Green), and all PIs were within the expected range (i.e., Green). Therefore, the NRC plans to conduct ROP baseline inspections at your facility.
In accordance with Inspection Manual Chapter 0305, Operating Reactor Assessment Program, the NRC identifies cross-cutting issues (CCIs) to communicate a concern with the licensees performance in a cross-cutting area and to encourage the licensee to take appropriate actions before more significant performance issues emerge. In the previous end-of-cycle assessment letter for Quad Cities Nuclear Power Station (ML24059A007), dated February 28, 2024, the NRC identified a cross-cutting theme where six findings were identified in the assessment period that demonstrated a trend in human performance issues associated with the aspect of conservative bias (H.14). Based on site performance from January 1, 2024, through December 31, 2024, the NRC has determined that a CCI now exists because this is the third consecutive occurrence of this cross-cutting theme. Although you have taken some actions to address this CCI, these actions have been unsuccessful in arresting this negative trend over the past three assessment cycles. Therefore, this CCI will remain open until: (1) you have completed an evaluation identifying the causes of the behaviors and developed a plan to address the safety culture behavior underlying the cause(s) of the CCI. This plan shall be

inspectable and provide a method for monitoring the sites overall progress in this area; and (2)
The NRC will perform a future inspection sample under Inspection Procedure 71152, Problem Identification and Resolution, to determine whether this evaluation demonstrates a reasonable understanding of the cause(s) of the CCI, has clearly defined corrective actions to address the cause(s), and results in an improving trend in the number of findings with the same cross-cutting aspect and/or leads to increased confidence in the ability to address the CCI over the remainder of the assessment period.
The enclosed inspection plan lists the inspections scheduled through December 31, 2026.
The NRC provides the inspection plan to allow for the resolution of any scheduling conflicts and personnel availability issues. Routine inspections performed by resident inspectors are not included in the inspection plan. The inspections listed during the last 12 months of the inspection plan are tentative and may be revised. The NRC will contact you as soon as possible to discuss changes to the inspection plan should circumstances warrant any changes.
This letter and its enclosure will be made available for public inspection and copying at ADAMS Public Documents l NRC.gov and at the NRC Public Document Room in accordance with Title 10 of the Code of Federal Regulations 2.390, Public Inspections, Exemptions, Requests for Withholding.
