NOTICE OF VIOLATION Duka Power Company Docket No. 50-413 Catawba 1 License No. NPF-35 During the Nuclear Regulatory Commissio, (NRC) inspection conducted en May 26 -

July 8,1987, a violation of NRC requirements was identified. The violation involved a failure to determine the cause of a valve inappropriately remaining shut rendering a channel of containment pressure inoperable.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is listed below:

Technical Specification 6.8.1 requires that written procedures shall be established, implemented, and maintained covering the applicable proce-dures recommended in Appendix A to Regtilatory Guide 1.33, Revision 2, February 1978.

Duke Power Company Safety Review Group Incident Investigation and Report Preparation, SR3/2, and Catawba Nuclear Station Directive 2.8.1.

Problem Investigation Process and Regulatory Reporting collectively require reportable events to be thoroughly investigated, the cause determined, and the full implications of recurring events evaluated.

Contrary to the above, the licensee completed Incident Investigation Report C87-040-1 of May.14, 1987, and did not theroughly investigate, determine the cause, and evaluate the full rect.rring event implications for valve INSPT5040 inappropriately remaining shut from April 7 to April 24, 1987, causing Containment Pressure Channel IV to be inoperable.

This is a Severity Level IV violation (supplement I).

Pursuant to the provisions of 10 CFR 2.201, Duke Power Company is hereby required to submit to this Offi;e within 30 days of the date of the letter transmitting this Motice a written statement or explanation in reply including:

(1) admission or dental of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further viola-tions, and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
