' Gentlemen:

SUBJECT: NOTICE OF VIOLATION

(INSPECTION REPORT NOS. 50-413/87-20 AND 50-414/87-20)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

P. K. VanDoorr., and M. S. Lesser from May 26, 1987 to July 8, 1987. The

inspection included a review of at:tivities authorized for your Catawba

facility. At the conclusion of the inspection, the findings were discussed

with those members of your staff identified in the anciesed inspection report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective ruminations of procedures

and representative records, interviews with parsornel, and observation of

activities in progress.

The inspection findings indicate that certain activities appeared to violate

NRC requirements. The violation, references to pertinent requireinents, and

elements to be included in yeur response are presented in the enclosed Notice

of Violation.

In addition to the need for corrective actidn regarding the specific matter

identified in the enclosed Notice, we are concerned about your management

rev. lev process which allowed a repetitive probicm to be investigated without

expending sufficient effort and expertise to find the root cause.

Consequently, your response should describe those particular actions taken or

planned to improve the effectiveness of your program.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this lettu and its enclosures

will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosures are not subject to the
