NOTICE OF VIOLATION j North Carolina State University Docket No. 50-297  ;

Pulstar Reactor License No. R-120 During the Nuclear Regulatory Connission (NRC) inspection conducted on September 29-30, 1987, a violation of NRC requirements was identified. The violation involved failure to maintain and follow radiation control procedures.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is listed below:

Technical Specification 6.3.a.8 states that operating procedures shall be written, updated periodically and followed for radiation control, and shall be maintained and available to all operating personnel.

Procedure HP-40-1, Utilization of Reactor ! m dfation Facilities, Revision 1 January 7,1986,- Paragraph 8 states that the stringer is placed' on the Bridge for dose rate measurement which shall be provided to the Reactor Operator for entry on Fonn NRP-3 (Run Sheet).

Paragraph 6 of the above procedure states that Reactor Operations will be notified before removing anything from the pool.

Contrary to the above, operating procedures for radiation control were not written and/or followed in that:

1. Between June 22 and September 29, 1987, dose rate measurements were not entered on the Run Sheets for approximately 90 percent of the stringers removed from the reactor pool.
2. Between June 22 and September 29, 1987, Reactor Operations was not notified prior to removing stringers from the reactor pool.
3. The methodology used for calibration of personnel contamination monitor (RM-14s with HP-210 pancake probes) was not maintained in a written procedure.

This is a Severity Level IV violation (Supplenent IV.

Pursuant to the provisions of 10 CFR 2.201, North Carolina State University is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further 8710150578 871009 PDR G ADOCK 05000297 PDR

- violations, and (5). the date when full compliance will be achieved. Where good cause is-shown, consideration will be given to extending the response time.
