Preface r~
This subcategory report is one of a series of reports prepared for the Employee Concerns Special Program (ECSP) of the Tennessee Valley Authority (TVA). The ECSP and the organization which carried out the program, the Employee Concerns Task Group (ECTG), were established by TVA's Manager of Nuclear Power to evaluate and report on those Office of Nuclear Power (ONP) employee concerns filed before February 1, 1986. Concerns filed after that

date are handled by the ongoing ONP Employee Concerns Program (ECP).

l The ECSP addressed over 5800 employee concerns. Each of the concerns was a formal, written description of a circumstance or circumstances that an l
employee thought was unsafe, unjust, inefficient, or inappropriate. The I
mission of the Employee Concerns Special Program was to thoroughly J
investigate all issues presented in the concerns and to report the results of those investigations in a form accessible to ONP employees, the NEC, and the general public. The results of these investigations are communicated
.1 by four levels of ECSP reports: element, subcategory, category, and final.
Element reports, the lowest reporting level, will be published only for those concerns directly affecting the restart of Sequoyah Nuclear Plant's reactor unit 2.
An element consists of one or more closely related issues. An issue is a potential problem identified by ECTG during the evaluation process as having been raised in one or more concerns. For l
efficient handling, what appeared to be similar concerns were grouped into elements early in the program, but issue definitions emerged from the i
evaluation process itself. Consequently, some elements did include only l
one issue, but often the ECTG evaluscion found more than one issue per element.
Subcategory reports summarize the evaluation of a number of elements.
)
Jowever, the subcategory report does more than collect element level evaluations. The subcategory level overview of element findings leads to an integration of information that cannot take place at the element level.
This integration of information reveals the extent to which problems overlap more than one element and will therefore require corrective action l
for underlying causes not fully apparent at the element level.
To make the subcategory reports easier to understand, three items have been placed at the front of each repor t:
a preface, a glossary of the j
termino 1cgy unique to ECSP reports, and a list of acronyms.
l Additionally, at the end of each subcategory report will be a Subcategory Summary Table that includes the concern numbers; identifies other subcategories that share a concern; designates nuclear safety-related, safety significant, or non-safety related concerns; designates generic applicability; and briefly states each concern.
Either the Subcategory Summary Table or another attachment or a combination of the two will enable the reader to find the report section or sections in which the issue raised by the concern is evaluated.

intimidation, harassment, and wrongdoing will be released by the TVA Office of the Inspector General.
Just as the subcategory reports integrate the information collected at the element level, the category reports integrate the information assembled in all the subcategory reports within the category, addressing particularly the underlying causes of those problems that run across more than one subcategory.
A final report will integrate and assess the information collected by all of the lower level reports prepared for the ECSP, including the Inspector General's report.
For more detail on the methods by which ECTG employee concerns were evaluated and reported, consult the Tennessee Valley Authority Employee Concerns Task Group Program Manual. The Manual spells out the program's objectives, scope, organization, and responsibilities. It also specifies the procedures that were followed in the investigation, reporting, and j

1.1 Introduction The Miscellaneous subcategory consists of those concerns related to

management and personnel that could not be assigned to any other management and personnel subcategory. Because of the wide variety of the concerns addressed, the format of this report deviates somewhat from the customary structure.
I The evaluator has reviewed all the information available on the conearns in this subcategory. The information pertinent to the l
evaluation of the issues has been considered and incorporated in i
this report.
Because each concern has been evaluated as a separate issue, the characterizations, causes, and corrective actions for each issue are presented with the findings on that issue in section 4.0.
This approach was taken to minimize the fragmentation that would result if each issue was discussed in each section.
1.2 Description of Issues The Miscellaneous subcategory includes 48 individual concerns.
Thirty-four (34) of these concerns expressed general opinions which were evaluated as separate issues.
Fourteen concerns did not clearly express a concern; just what the employee felt was unsafe, unjust, or inefficient could not be determined. Consequently, these concerns could not be evaluated due l
t to insufficient information.

The evaluation for this subcategory was conducted in accordance with the
" Evaluation Plan for the Employee Concerns Task Group" and the
" Evaluation Plan for the Management and Personnel Group" and the
" Miscellaneous Subcategory Evaluation Plan."

A review was conducted of all available files associated with the individual concerns, including Office of Employee Relations and Nuclear Safety Review Staff (NSRS) information Previous investigation reports i
of nine concerns evaluated by line management were reviewed and the

information was used in the preparation of this report.

Interviews were held with members of the other six Category Evaluation d
Groups (CEGs) of the Employee Concerns Task Group (ECTG) regarding the miscellaneous concerns that may be related to their areas of evaluation.
Discussions were also held with other evaluators within the Management and Personnel Group to discuss the concerns that may be related to their i

4.0 FINDINGS 4.1 Issue 71201 - Weste of Time and Paper The Concerned Individual (CI) contends that it is an unnecessary waste of time and paper to copy every Nuclear Dispatch or any cther publication for every employee on site (at present 3000 copies of each). These copies are discarded throughout the site and must be continuously picked up.

Much paper is used for the Nuclear Dispatch and similar publications. However, one of the major comitments of the new Office of Nuc1(ar Power (ONP) management is effective communication between management and employees. The Nuclear Dispat_eh is one i
1 important tool for bringing about this improved communication.
While wide distel5ution of the Nuclear Dispatch may seem wasteful, it is a cost effective means of achieving better communication.
Therefore, this issue is not factually accurate.

The CI contends that Tennessee Valley Authority (TVA) upper i
management appears to be in a state of confusion.
The TVA Board of Directors, recognizing a need to add stability and I
leadership to TVA's nuclear program, has contracted with a new ONP manager. It is the intent of the new ONP manager to provide stability and effectiveness and restore employee confidence, i

4.3 Issue 71203 - Telephone Wiretapping The CI contends that telephones are being " bugged" by TVA.
One instance of phone " bugging" at the Watts Bar Site was investigated by the Office ef the Inspector General (OIG) and found i
l to be valid. Personnel av :on was taken against the en.ployees involved in the unauthorized act. No further evidence of phone
" bugging" was identified by this investigation. If employees have information about any other concerns of this type, they are encouraged to contact the OIG.
l Is_ sue 71204 - Rapid Changes in NRC Regulations 4.4 s
The CI contends that rapid changes in Nuclear Regulatory Commission (NRC) regulations required repeated modifications to plant systems.
l NFC regulations are changing less frequently than in the past.
It is the interpretation of regulations by both TVA and the NRC that i
leads to system modifications. The NRC also informa utilities of i
potential problem areas encountered at any facility which might l
affect safety standards at other plants. Many changes were j
initiated as a result of accidents'in the industry. This issue is t
factually accurate but is not a problem requiring corrective action by the Office of Nuclear Power.
4.5 Issue 71205 - Harassment by IFS I
The CI contends that TVA allows IRS to take money from workers on j
this project without due process of law.

The Internal Revenue Code establishes TVA's Tax withholding obligation. As a Federal agency with significant tax-funded programs, TVA must support the laws and agencies' responsible for managing and enforcing the nation's tax laws. Therefore, this issue is not factually accurate.
4.6 Issue 71206 - Weldinz Recualification Test The CI contends that he/she was forced to take the welding requalification test, though CI did not want to.
A decision was made by ONP management that, in certain areas', all employees involved with welding would be refortified. This issue is factually' accurate but is not a problem requiring corrective action.

4 4.7 Issue 71207 - Spent Nuclear Fuel
'10; The CI contends that the nuclear industry has not made adequate provisions to handle and process spent nuclear fuel.
The problem of nuclear spent fuel managsment is certainly not plant or site-specific. It is also.a problem that has been recognized for several years and has required considerable 9 expense and replanning by the nuclear industry as 'arious Federai prodrF.ms and policies v
have changed directions; but it is perhaps closer to solution with the passage of legislation in 1982 to develop e geologically safe repository.
Until the Department of Energy (DOE) accepts spent fuel for disposal, utilities are responsible for storing it.
The NRC hae-determined that utilities can safe 2y store spent l'uel onsite for 50 years or more. TVA's current plan is to make full use of its capacity for storage inplant'in spent' fuel pools. This method will be augmented by onsite dry storage in large metal casks, if necessary. Therefore, this issue is not factually accurate.
4.8 Issue 71208-Nuclear Safety at WBN' The concern that raised this issue was only a comment on nuclear safety at WBN "I don't trust this one."
i One of the primary objectives of the employee concerns efforts at I
WBN is to validate the safety of the plant and to bolster employee i
confidence in that safety. This issue could not be verified as factually accurate.

4.9 Issue 71209 - Location of WBN The CI contends that the WBN plant shou 1Ld not have been built beside Watts Bar Dam.
I Indepth engineering and geological studies were mandatory before a 5.
decision could be made on the location of WBN. Those studies concluded that WBN would not endanger the safety of the Watts Bar p
Dam. Nor was the location of the dam found to be a hazard to the

safe operation of WBN. Therefore, this issue is not factually accurate.
4.10 Issue 71210 - Plot Aasinst Section Supervisor This issue was raised by a CI who overheard two individuals (names known) plotting to plant a bottle of liquor in the section
(
supervisor's (name known) desk in an effort to have him terminated. CI discussed the incident with a co-worker (name known) on June 7, 1985, and desired the Employee Response Team I
(ERT) to make a confidential record of the expression of the incident, should the individuals carry out their plans.
This issue could not be verified es factually accurate; however, as a precaution, the concern has been permanently recorded in the data

files of the NRC, who obtained copies of all WBN employee concerns collected by Quality Technology Company (QTC). This documentation will provide evidence should such t. Plot be discovered.
l 4.11 Issue 71711 - Forolan Material Purchases The CI contends thst TVA should not order materials from foreign countries but should support U.S. manufacturers. An example given was the purchase of heat exchangers from Belgium.

TVA determines whether a domestic supplier's bid is unreasonable as compared with the bid of a foreign supplier by applying the provisions of Executive Order 10582..In cases where foreign purchases are made under the'se provisions, significant savings for the users of TVA power result. Therefore this issue is not factually accurate.

4.12 Issue 71212 - Leave Time Without Documentation The CI contends that an employee took a large amount of leave time without completing the required documentation.
Without proper evidence, this concern could not be verified as factual. However, if an individual has knowledge of an employee abusing TVA leave privileges, the individual should notify the employee's supervisor and let the supervisor take the appropriate action.
4.13 Issue 71213 - Warehouse Gate Shack The CI raising this issue stated there should be a shack'at the.
main warehouse gate. Present' procedure requires drivers to stop at gate, walk over 100 feet to.the warehouse, sign in, walk back to the vehicle, wait for the individua1' assigned to the gate to open it, and then drive through. This procedure is reversed when leaving. CI stated that this is not very cost effective.
This issue was verified as factual but it was previously resolved in June 1986 with the construction and operation of a gatehouse at the main warehouse gate.
4.14 Issue 7121a - TVA Employee Persecution The CI alleges thet "TVA will use any pretest to persecute i
employees."
This type of perception on the part of some TVA employees is what led to the development of the Employee Concerns Program. There are several avenues employees can take if they feel they are being unjustly treated; redress can be sought from line supervision, the grievance process, the office of the Equal Employment Opportunity.

The CI. contends that, instead of making every effort to retain qualified Operations personnel, TVA cooperates with and encourages other utilities who want to hire away TVA Operations personnel by l
providing facilities for recruiters to interview the TVA
' employees.
This issue is factually accurate, but is not a problem.-- The only time TVA has provided space for recruiters is when TVA was having a large reduction in force and'was trying to assist employees needing placement outside TVA.
'4.16 Issue 71216 - Instrumentation Engineering Accountability-The CI contends that Instrumentation Engineering is not held responsible for mistakes they cause. Exemples given were (1) Instrumentation lines with incorrect slope were approved by g
Engineering, (These.are now being addressed by Nonconforming-l Condition Reports (NCRs).) (2) The Engineering deletion of stainless steel cleanline requirements for unit 1, even though this f

4.30 Issue 71230 - Supervision of Pipe Leading and Unloading j
This issue is based on a complaint that an expeditor of material cannot move pipe until a general foreman can watch loading and unloading of the pipe. This allegedly creates a schedule impact due to the busy schedule of a general foreman.
This procedure was established in support of safety. A large piece of pipe rolled off a truck onto a man, injuring his leg. Since then, general foremen are required to oversee all unloading of pipe to ensure safety is practiced. Therefore, this issue is factually-accurate but is not a problem.
l 4.31 Issue 71231 - TVA Weldina Program The CI raising this issue stated that, if TVA had an adequate welding program, then the welders would not have lost two or three weeks time.

CI also stated that quality of nonsafety-related systems is not as good as quality of safety-related systems.

However, informal (practice) oral exams are given at the plants to help prepare the employees for the formal exam. 4.34 Issue 71234 - Bellefonte Completion The CI raising this issue states that "TVA should finish the nuclear plant that is built the safest so far and has complied with the procedures. The community's feeling should be considered."
