TMs incident, which scearred cm AprG 3, 1960, was r ported to the NY Inspection Division and the Division of Inspecties, HQ, j by telephone sa AprG 4, 1960. A written anoamer==d=== froma l Westinghouse to the Director, Division d Licensing and Regulation was received om April 7, 1960 (a copy of which was farmished you).

A press release was issued by Westinghouse on Apra 4, 1960 and the JCAE was informed by phone on this date.

Further informaation, developed by the Inspection Division, NY, indicates that this incident should be classified as a Type A Radiation heident, under AEC 0707-053, in that the lost tiene, as a direct resmit of the insident wal probaMy esseed one week sad that the cost wiu probably exceed $100,000.
