RANCHO SEC0 WIRE AND CABLE PROGRAM (TAC NO. 65195)

The purpose of this letter is to identify for you the scope of wire and cable related activities the staff would like to focus on durir.g the site visit planned for the week of June 15, 1987. We expect that you will use this information to prepare for the meetings and inspection activities.

As discussed with Mr. Jim Gough of your staff the NRC inspection team will consist of at least one person from NRR (Mark Caruso), one person from the NRC Region V office (Bill Ang) and an NRC Contractor (George Morris). The purpose of their visit is to gather information regarding the root cause of the cable installation problems identified at Rancho Seco and your corrective action program. The information will be used to prepare sections of the staff's Rancho Seco restart safety evaluation.

As you are aware, the staff has expressed its concerns and interests regarding resolution of wire and cable issues in the May 1, 1987 letter from John Martin and during our meeting on May 6, 1987. The particular issues that the staff wishes to focus on during the upcoming visit are outlined in Enclosure 1 to this letter.

Enclosure 2 to this letter lists the specific task items from your action plan (Enclosure to June 1, 1987 letter from G. Andognini) which in our view should yield responses to our questions in Enclosure 1. Therefore, we would like to meet with cognizant personnel and discuss these specific items during our visit.

We would like the meetings to be informal, small group, interview type sessions.

Complete written responses to the questions are not required at the time of our visit. However, we request they be provided no later than July 31, 1987.

Enclosure 3 to this letter provides a list of specific inspection activities we wauld like to accomplish while on-site.

' ENCLOSURE 1 l NRC REQUEST FOR INFORMATION REGARDING CABLE INSTALLATION PROBLEMS

1. Describe the installation procedures and practices which have been used at Rancho Seco to ensure that licensing criteria for safety related class IE and Appendix R safe shutdown and associated cable are met. Your response should cover original cable installation performed by Bechtel as well as new cable installation and re-routing which has taken place since the plant was put into operation. The response should address the following specific topic areas:
a. Provide the detailed sequence of events involved in the design and installation of new cable and re-routing of previously installed cable.
b. Provide the Quality Assurance Procedures applicable to control of cable design, installation and re-routing; including verification, quality record control, review and approval authority and management oversight,
c. A clear description of the significant changes which have taken place in either installation or quality assurance procedures (e.g.,

change in computer tracking systems from EE553 to CRTS).

2. Based on the response to question 1 and the results of IAG Group investigations of the cable routing errors provide a clear description of the events and circumstances which led to:

a. The misrouting of cable described in LERS 85-16 Rev. 1, 86-10, 87-13, 87-26 and that discovered recently during implementation of the sampling plan.
b. The discrepancies between the cable routing in the plant and that contained in the quality assurance records and Cable Raceway Tracking System.
