i 1.0 PURPOSE i

This document provides details concerning SMUD's program to evaluate the adequacy of the Cable and Raceway installation at Rancho Seco including the. adequacy of the Computerized Cable and

- Raceway Tracking system (CRTS). The SMUD actions are referred to in this document as the Action Plan.

l 2.0 SCOPE The scope of this document includes the evaluation of all scheduled cable and raceway installed at Rancho Seco.

" Scheduled" Cable and Raceway is all Cable and Raceway installed

at Rancho Seco Nuclear Generating Station (RSNGS) and recorded in~

and controlled by the CRTS. The only cable and raceway

! designated as " unscheduled" (and not included in the CRTS) is i wire.and raceway installed in separate conduit systems dedicated to systems such as lighting and telephone wiring.

This program identifies and addresses the resolution or action 1 plan for the following con. erns:

1. Do the CRTS recorded cable locations match the "As-Built" j cable locations?

3.0 BACKGROUND

1 The principal concerns initiating SMUD Actions are listed as 1

references 1, 2 and 3. ODR 86-125 resulted from an employee allegation to the SMUD Ombudsman (Mr. Carl Andognini) concerning I cable tray overfills and the validity of the CRTS (Cable and Raceway Tracking System) Data Base.

Initial action taken by SMUD was to initiate enhancement of the i CRTS Program and to use the enhanced program to check for Raceway overfills, violations of separation criteria, intemixing of instrument cables with power and control cable, and other

discrepancies as listed in Sections 4.4 and 4.5. Although a number of discrepancies were " Bookkeeping" problems, generated by the enhancement, a significant number appeared to be of real I

concern. The existence of known problems in LER 85-16, ODR i- 86-125 and LER 86-10 raised questions concerning:

1) The degree of control exercised on the. cable installation.

,2) The accuracy and completeness of the CRTS Data Base.

l Late in 1986 a decision was made to signal trace a significant i sample of safety cables installed from 1975 through 1986 and 4 compare the "As Built" condition with plant documents and the i CRTS Data Base. The sampling plan and methodology is summarized in Section 4.2.2 and detailed in Reference 4.

Early in January the various on-going activities were integrated

into a five part " Wire and Cable Action Plan" under a single -

j . program manager.

4.0 WIRE AND CABLE ACTION PLAN

} 4.1 Resolution of Known Physical Problems

The known problems and the ~ status of each are listed in Attachment 4. This attachment lists all problems published as Occurrence Description Reports (ODR). Licensing Event Reports

, (LER) or Non Conformance Reports (NCR). Infonnation listed

includes dates and dispositions.

4.2 Resolution of Concerns Regarding The Location of Safety Cables

! Installed since Commercial Operation.

4.2.1 Sampling Plan THE CRTS Data Base has identified 2800 Safety related cables

! installed since the plant went into Commercial Operation in

! 1975. The numbers of non-safety cables (installed since j commercial operation) is not known with the same degree of I accuracy because the number of installed security cables and j communication cables has still to be defined.

. The location of the 2800 safety cables are being sampled to i establish a confidence level in the CRTS Data relative to cable 4

locations. The cables are listed and the samples selected from 1 the design drawings and not from the CRTS Data Base. This avoids reliance on the CRTS for developing the sample populations.

} The sampling methodology and degree of conformance to QAIP No. 3 j and MIL-STD-1050 are detailed in reference 4. The sampling plan 1 identifies 238 Safety Cables which have been randomly selected i

for signal tracing to verify the CRTS recorded cable route. As detailed. in Reference 4. the number of major defects will

establish the confidence levels for each of the 4 populations i selected for investigation.

.. . l A major defect for a signal traced cable would be a route which differed from the CRTS Data Base and also violates plant safety.

The decision process classifies three levels of defect through a detailed engineering review which utilizes a decision process illustrated with diagrams'in reference 4. These levels are:

1. Major Defect
2. Minor Defect-3.' Insignificant Defect 4.2.2 Development of Cable Population Lots t The following is a summary of the method by which the sampling program selected .the cable lots to be signal traced. Full detail is contained in reference 4.

The population lots for the Rancho Seco CRTS Sampling Program were developed based on reviews of CRTS related problems that were identified at the time of the program implementation. This review took into consideration known problems with the CRTS database, the following specific plant documents (LER 85-15, LER 86-10, ODR 86-125, and NCR S-5270), as well as interviews with Rancho Seco personnel (engineering, operations, and the CRTS staff).

A meeting, held on December 12, 1986, between SMUD, Impell, and MAC, settled on seven possible categories for the lots. These candidates for lots, to be used in the sampling program for CRTS, were selected based on the aforementioned review of CRTS related problems. They are as follows:

1. Safe Shutdown (SSD)/ Safety Related (SR) circuits installed prior to commercial operation.
2. SSD/SR circuits installed during the major outages in 1981, 1983, and 1985.
3. Cable pull cards with questionable signatures.
4. Appendix R circuits with fire protective wraps. l
5. SSD/SR circuits that were rerouted since conmiercial operation.
6. Class 2 Appendix R circuits.
7. SSD/SR circuits installed between 1974 and 1980.

From the above categories, with the exception of item 1, it was deteniined the information contained in items 2 through 7 could be consolidated in four homogeneous independent lots as defined in Impe11 Calculation No. 0790-201, Revision 3, Section 6.0 (Reference 4).

The populations for Lots 1 and 2 were initially organized chronologically by the year the cable was installed or routed.

Class 2 Appendix R circuits and circuits required to be provided with a fire protective wrap to meet Appendix R requirements, in general, were grouped together and included at the end of the chronological listing of circuits for each lot. as appropriate.

Lots 3 and 4 were fonned subsequent to the definition of Lots 1 and 2. The sample selection process proceeded as noted in Impe11 Calculation No. 0790-201, Revision 3. Section 8.0 (Reference 4).

The lot selection, the.crganization of the lots population, and the initial sample selection methodology were designed so as to yield the greatest amount of information regarding any potential defects that may be found during the circuit tracing process.

These results could then easily be evaluated to determine if a connon cause was surfacing as the result of any deviations found and the sample program expanded accordingly.

4.2.3 Personnel Training and Responsib111 ties Four two-man inspection teams were utilized at peak with additional personnel for supervision and recording and evaluating resul ts. All personnel were experienced and qualified and their names and resumes are on record.

Training Training was given and documented in two areas:

1) Use of Standard Test procedures STP-976 (Attachment #1) and useofStandardTestprocedureSTP-972(Attachment #2).
2) Use of the two signal tracing devices used:

o Radio Detection Corp. RD-600 Locator.

o Pacer Current Tracer Model CR-10.

Training for the RD-600 was by factory personnel and is documented. Training certificates are on record.

Additional instruction was given in the use of administrative procedures:

AP.3 Administrative Produce Work Requests AP-4A/B Safety Clearences/ Test Authorization procedures, respectively.

AP-22 Occurrence Description Reports.

l RESPONSIBILITIES l

Responsibility for the program is with the CRTS Program Manager l (Jim Gough). Direct day-to-day direction of this cable tracing effort is by the Deputy CRTS Program Manager (Bob Carroll).

Resumes for both are on record. The personnel for the Cable Tracing Work are supplied by Impe11 Corporation under Contract Tasks 271 and 334.

4.2.4 Walkdown (Cable Tracing) Procedures Procedures are Fully detailed in STP-976 (Attachment #1) and STP-972 (Attachment #2).

4.2.5 Reporting of Walkdown Findings The engineer on the two man cable tracing team records any deviations on the routing verification sheet. Deviations involving possible safety concerns are imediately reviewed by. .

the supervisor who verifys the deviation. An Occurrence l Description Report is then prepared for signature by the CRTS -

Program Manager or his Deputy. This process occurs within the I shift in which the Walkdown takes place.

Deviations of a minor nature are evaluated and the evaluation recorded. Attachment #3 is a typical evaluation record. This  :

evaluation assigns a classification of " Minor Defect" or

" Insignificant Defect". After review by the CRTS Program Manager ,

the evaluation is submitted to the Plant Review Committee (PRC) )

for review and concurrence. '

A summary of results through March 24, 1987 is given in section 4.2.6.

4.2.6 Results A confidence level of 95/95 (as detailed) will be accepted as  !

demonstrating that the CRTS Data Base is acceptable without 1 further signal tracing. l An unacceptable confidence level (less than 95/95) will require additional signal tracing to a level which will demonstrate a confidence level of 95/95.

The following table gives the results of the sampling program as of March 24, 1987. Signal tracing is expected to be complete by of April 30, 1987.

  • With the refonnation of Lots 1, 2, and 3 (due to the addition of Lot 4)

, one additional circuit in Lot 2 was traced and three additional circuits in Lot 3 were traced, but were not selected by the random sample. The

, total number of-circuits to be traced therefore, equals 238.

4.2.7 Exclusion of Original Cable Population (Pre 1975) From the Sample Plan The Cable and Raceway installation at Rancho Seco at plant turnover (1975) had been designed and installed by Bechtel Power Corporation (BPC). BPC was both the designer (Architect Engineer) and Constructor. The BPC design office was in Nomalk, Los Angeles while the construction was by Bechtel direct labor.

The computerized Circuit and Raceway program used was a  ;

proprietary program EE553 of BPC. The current program, named i CRTS (Cable Raceway Tracking System), is one written by Control Data Corporation (CDC) to SMUD's specifications during 1978 and l- 1979, and instituted in July 1980. .

.l s s < j The design, installation and quality control of, cables installed

- at Rancho Seco, prior to plant turnover, was centrolled by Bechtel Power Corporation Procedures. These BPC' Procedures were standard for BPC Projects of this vintage and were adequate to control and document the Cable and Raceway installation.

Documentation supporting this decision is being furnidhed by the

'Bechtel Power Corporation in the following:

g\; As of March 10, 1987 the two cables identified in LER 85-16 are the only known problems in the original cable population. As

,, identified this was a unique design error;not related to the type of problems under review. The possibility of a Generic Bechtel Design problem has been raised by the Plant-Review Committee and Region 5 and is being investigated. . Documentation, supporting this decision will be furnished when received and evaluated as previously described.

4.3 Resolution of Concerns Regarding'The' Completeness of the CRTS Data Base The quality concerns raised'concerning the completeness of the CRTS Data Base will be addressed as follows: '.

A review of procedures governing cable installation from commercial operation (1975) through 1986. This review will address questions such as:

(1) Did QCI No.107 result in the destruction o'T raceway and pull cards? How many?

(II) How many procedures existed (governing cable installation) in this period; How many SliUD entities j (Engineering Design Maintenance, etc) initiated cable l installation? j (III) Did authority exist to by-pass procedures and was this authority exercised? Were procedures by-passed and to what extent?

(IV) To what extent are security cables included in the CRTS Data Bank?

(V) To what extent are comunication and other cables included in the CRTS Data Bank?

which require action to complete the program enhancement, but which have no safety significance. The resolution of these CRTS Data Base Discrepancies has in its scope the entire cable population. The schedule for completing evaluations on these minor discrepancies will be provided when completed and is not a restart prerequisite. location verification activities for plant power and control cables.

2.0 REFERENCES

3.1 Nuclear Operations shall review plant impact statement on Enclosure 8.1 for concurrence.

3.2 The shift supervisor has been given a copy of this procedure. He shall determine the need for test tags depending on the requirements of the work request. The cable is shown on the cable data sheet, Enclosure 8.1.

3.3 The appropriate sections of Enclosures 8.1 and 8.2 shall be completed for inclusion in the Work Request package.

4 PREREQUISITES (Continued) 3.4 All personnel performing the cable routing verification are familiar with the following aspects of the circuit as listed on Enclosure 8.1.

3.8 The test equipment has been checked for proper operation in accordance with manufacturer's recommendations.

3.9 8efore the start of verification, the group leader has performed a walkdown of the area to visually check for. accessibility (ladder or scaffolding is provided, where necessary), location of equipment and power supplies, and any unsafe or abnormal conditions.

3.10 Test Engineer to ensure that signature log on Enclosure 8.1 completed.

4.0 TEST EQUIPMENT Each test set includes a volt-ohmmeter (VOM), a circuit tracer with its accessories such as flex hook-up wiring with alligator clips, and an 18-volt to 24-volt battery. Three types of circuit tracers can be used in this STP, as follows:

4.1 Pasar Current Tracer 4.1 .1 Model CT-10: For deenergized circuit tracing, use model CT-10.

transmitter, model P-23 probe, and an 18-volt to 24-volt' battery.

See Enclosure 8.3, Figure 1, for connection diagram.

4.2 RF Open-Circuit Tracer 4.2 .1 Pasar RF Tracer Model OT-1000 This includes model OT-1000 transmitter and model P-1000 probe, SRC 15 signal return cable, and EP-1000 ear ~ piece. This instrument.

will operate-on. a deenergized ungrounded circuit only. The output power is 10mW maximum, 445 kHz carrier frequency, and ,1.7 kHz audio modulation frequency. ' See Enclosure 8.3, Figure 2, for. connection diagram.

4.3 RF Multi-Frequency Tracer 4.3 .1 Radiodetection Model RD-600 Tracer equipment has been designated "No Calibration

The RD-600 transmitter / receiver has four different- frequencies: 1, 8, 33, and 130 kHz. The frequencies to be used in these tests will be 1 or 8 kHz. See Enclosure 8.3, Figure 3, for connection diagram.

5.0 LIMITS AND PRECAUTIONS 5.1 Verification of cable routing shall be performed only during cold shutdown.

5.2 Only circuits of one train of the redundant systems shall be tested at the same time.

5.3 For cables routed in common raceways or in raceways close to each other, tracing shall be performed using different frequencies or uniquely identified audio signals (continuous or pulse-mode).

6.0 PROCEDURE NOTE: A test log shall be kept on Enclosure 8.4 NOTE: Signatures.for.all procedural steps must be signed on Enclosure 8.1.

6.1 All the prerequisites have been initialed and dated.

6.2 The engineer will determine the circuit information from the wiring diagrams for deenergizing the circuit-before testing. The routing of the cable shall be determined from the CRTS routing data and the appropriate raceway drawings.

6.3 Cable routing verification.

location verification activities for plant instrumentation cables.

PREREQUISITES (Continued) 3.4 All personnel performing the cable routing verification are familiar with the following aspects of the circuit as listed on Enclosure 8.1.

3.7 Training in the use of test equipment has been conducted and the walkdown team members are proficient in the use of the required equipment.

3.8 The test equipment has been checked for proper operation in accordance with manufacturer's recommendations.

3.9 Before the start of verification, the group leader has performed a walkdown of the area to visually check for accessibility (ladder or scaffolding.is provided, where necessary), location of equipnent and power supplies, and any unsafe or abnormal conditions.

3.10 Test Engineer to ensure that signature log on Enclosure 8.1 completed.

4.0 TEST EQUIPMENT Each test set includes a volt-ohmmeter (V0M), a circuit tracer with its accessories such as flex hook-up wiring with alligator clips, and an 18-volt to 24-volt battery. Two types of circuit tracers can be used in this STP, as follows:

4.1 Pasar Current Tracer 4.1 .1 Model CT-10: For deenergized circuit tracing, use model CT-10 transmitter, model P-23 probe, and an 18-volt to 24-volt battery.

See Enclosure 8.3, Figure 1, for connection diagram.

4.2 RF Multi-Frequency Tracer 4.2 .1 Radiodetection Model RD-600 ,

NOTE: Tracer equipment has been designated "No Calibration

. Required" by the Calibration Lab.

The RD-600 transmitter / receiver has four dif ferent frequencies: 1,

' - 8, 33, and 130 kHz. The frequencies to be used in these tests will

. be 1 or 8 kHz. See Enclosure 8.3, Figure 2, for connection diagram.

5.0 LIMITS AND PRECAUTIONS 5.1 Verification of cable routing shall be performed only during cold shutdown.

5.2 Only circuits of one train of the redundant systems shall be traced at the same time.

5.3 For cables routed in common raceways or in ar'ceways close to each other, tracing shall be performed using dif ferent f requencies or uniquely identified audio signals (continuous er pulse-mode).

l 14 5.4 Each circuit shall be deenergized before access to cable and electrical equipment and hook-up of the trace equipment.

5.5 No test equipment shall be connected until the "from" and "to" leads of r the instrument cable have been lifted at the tenninals. An exception will be rede in the case of devices in which the cable connection at the device end is difficult to unterminate, as in the case of RTDs and thermocouples. Provided the device is nonelectronic, hooking up the transmitter without disconnecting the cable at the device end, is e acceptable.

5.6 After verification of cable routing, the circuit being verified shall be restored to its original configuration. Wiring diagrams or other documents depicting the verified connection will be utilized to unterminate and reterminate. Conductor markers will be provided if none exist.
