. HIGHLIGHTS OF WEEKLY INFORMATION REPORT llEEK ENDING AUGUST 1, 1986 <

Millstone Unit 1 At 1905, July 23,1986, the Reactor Water Cleanup (RWCU) System experienced a pipe weld failure. Approximately 2200 gallons of reactor water were discharged to the Reactor Building. There was no offsite release. The failu're occurred at a weld connection to a 90 degree elbow on the 1" relief line for the "C" regenerative heat exchanger. There was complete separation of the relief valve from the pipe, but the relief valve remained attached to its support.

The first indication was a control room alarm indicating high temperature in the RWCU heat exchanger room. Upon investigation and report by a plant operator, the control room operator isolated the RWCU system. Analysis of the weld failure is in progress. Reactor power level, initially at 80% while investigating main condenser levels, continued unaffected'by the event. The RWCS remained isolated for about 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> while the area was cleaned up and the pipe repaired.

Power level is now at 100%.

Pilgrim Nuclear Power Station On July 25, 1986, Boston Edison announced that the current outage of Pilgrim '

Station will be extended into 1987 in order to complete the Appendix R fire protection modifications, to refuel the reactor, and to enhance its Mark I containment. This outage began on April 12 and the plant was expected to be ready for restart by September.

San Onofre Nuclear Generating Station On July 26, 1986, at 6:45 PM PDT, San Onofre Nuclear Generating Station Unit 1 closed to the grid and began power operation for the first time since the loss of power / water hammer event that occurred on November 21, 1985. The plant is currently at 30% power while doing flux mapping. The utility plans to increase power to about 90% by August 1, 1986.

On July 24, 1986, members of HMSS, SP, OGC, and IE met with Mr. Patrick Stevens, Nuclad, Inc., Denver, Colorado, to discuss what information would be Nuclad, needed for NRC approval to transfer ownership of a licensed facility.

lleekly Information Report Division of Contracts ADMINISTRATIVE MATTERS As reported in the Weekly Information Report for the week ending July 25, 1986, the Small Business Administration (SBA) determined that an offeror did not qualify as a small business under Solicitation No. RS-0RM-86-264, a competitive requirement set-aside exclusively for participation of small business. On July 28, 1986, the offeror filed a Notice of Appeal with SBA's Office of Hearings and Appeals seeking a reversal of that decision. SBA has advised the Contracting Officer that a decision on this matter would be made in approximately thirty days.

In a separate action on July 28, 1986, the offeror filed a protest with the General Accounting Office (GA0) asserting that the Contracting Officer's (CO) decision to exclude its proposal from further consideration under this acquisition was improper. On July 29, 1986, GA0 advised the C0 that the protest had been dismissed and that written confirmation of this decision would be provided to the C0 shortly.

The failure occurred at a weld connection to a 90 degree' elbow on the 1" relief line for the "C" regenerative heat exchanger. There was complete separation of the relief valve from the pipe, but the relief valve remained attached to its support.

The first indication was a control room alarm indicating high temperature in the RWCU heat exchanger room. Upon investigation and report by a plant operator, the control room operator isolated the RWCU system. Analysis of the weld failure is in progress. Reactor power level, initially at 80% while investigating main condenser levels, continued unaffected'by the event. The RWCS remained isolated for about 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> while the area was cleaned up and the pipe repaired.

Power level is now at 100%.

Pilgrim Nuclear Power Station On July 25, 1986, Boston Edison announced that the current outage of Pilgrim Station will be extended into 1987 in order to complete the Appendix R fire protection modifications, to refuel the reactor, and to enhance its Mark I containment. This outage began on April 12 and the plant was expected to be ready for restart by September.

l San Onofre Nuclear Generating Station On July 26, 1986, at 6:45 PM PDT, San Onofre Nuclear Generating Station Unit 1 closed to the grid and began power operation for the first time since the loss of power / water hammer event that occurred on November 21, 1985. The plant is currently at 30% power while doing flux mapping. The utility plans to increase power to about 90% by August 1, 1986.

Limerick Nuclear Power Station The Atomic Safety and Licensing Appeal Board issued a decision (ALAB-840) on July 18,1986 denying Robert L. Anthony's June 25, 1986 petition to reopen the offsite emergency planning hearing record. Mr. Anthony petitioned on the basis that a paper authored by a member of the licensee's legal counsel and published by the Washington Legal Foundation (WLF) was improperly provided to NRC officials. Mr. Anthony contended that this was on an ex parte communication, that it had prejudiced and discredited the NRC hearings on offsite emergency planning and that the licensee's counsel should be disqualified and the hearing record reopened.

The ASLAB denied the petition noting, among other things, that its most recent decision regarding Limerick, ALAB-836 dated May 7, 1986, was issued before any member of the ASLAB saw or was made aware of the WLF paper.

River Bend Station On July 10, 1986, a financial prospectus supplement was issued on behalf of the River Bend Licensee, Gulf States Utilities (GSU) to support a $100 million first mortgage debt issue. The prospectus reviewed the financial situation of GSU including adverse rate developments in the States of Louisiana and  !

Texas, retention of bankruptcy counsel by GSU and recently lowered financial l credit ratings by the major :ating services. The prospectus stated in part that "The circumstances referred to above could result in the company being rendered unable to pay dividends on its ... stocks, to meet its sinking fund  ;

obligations on its preferred stock, to continue making capital expenditures l which are primarily in connection with River Bend Unit 1, ..."  ;

The staff had earlier discussed the financial situation with the licensee and  !

had been verbally assured that the licensee would continue to make all necessary expenditures to ensure safe operation of the plant. The Senior Vice President ,

of River Bend Nuclear Group is scheduled to meet with the Director, NRR on '

July 31, 1986, for the purpose of discussing this and other matters.

Ho;e Creek Generatina Station l

A license authorizing full power operation of Hope Creek was issued Friday, July 25, 1986, to Public Service Electric & Gas Company (PSE&G) and Atlantic 1 City Electric Company. PSE&G will operate the single-unit facility. Hope Creek is owned by PSE&G and the Atlantic City Electric Company (95 and 5  !

percent ownership, respectively). The facility is located adjacent to the i licensee's Salem Generating Station on Artificial Island in Lower Alloways '

Creek Township, Salem County, New hrsey. Hope Creek is the last BWR with a Mark I containment to be lice n in the United States.

South Texas Project, Units 1 & 2 '

The Chairman of the ASLB on South Texas has written a letter dated July 25, 1986, to the attorneys for the Applicant requesting information on the Prompt l

Notification System related to Emergency Planning. The letter poses six questions to the Applicant predicated on "a prima facie basis for concluding 1 that the proposed Prompt Notification System may not comply with applicable l standards and hence constitutes a serious safety question sufficient to warrant l consideration under 10 CFR 2.760 a." Nine Mile Point 2 In a letter dated July 18, 1986, Mr. T. E. Lempges, Vice President for Nuclear Generation for Niagara Mohawk Power Corporation, stated that, on the basis of the current progress of its preoperational testing program and surveillance testing activities, the currently targeted fuel load date for Nine Mile Point Unit 2 is August 15, 1986. The staff believes a date of late August or September is more likely.

Ginna On July 29, 1986 at 0400, there was a rupture in an elbow in the drain line between the 2A moisture separator reheater and the 5B feedwater heater located in the turbine building basement. The sound was heard in the control room. The operators investigated and observed steam in the turbine building. They promptly initiated manual turbine and reactor trips, simultaneously, and closed the main steam isolation valves. There was no damage to safety related equipment, no release of activity, and no personnel '

injuries. The licensee has observed a 3 to 5 hole in the elbow. The plant is being brought to cold shutdown. The licensee plans to inspect all similar elbows prior to restart.

Limerick 4

On July 21, 1986 the ASLB scheduled a hearing for August 18 and, if needed, August 22, 1986 in Philadelphia, PA. The hearing will consider the offsite emergency planning issues remanded to the Board by the Appeal Board in ALAB-836, namely the availability of school bus drivers for two school districts.

Maintenance and Surveillance Program The Division of Human Facttes Technology has completed Phase I of the Maintenance and Surveillance Program (MSP) as described in SECY-85-129.

The objective of Phase I was to survey current maintenance practices in the nuclear industry and evaluate their effectiveness. Principal findings are: 1) While overall measured industry perfkraance trends are positive, there is a need to improve performance of maintenance and i

surveillance in order to reduce challenges to safety systems and to improve safety systems availability, 2) There is a wide variability in plant maintenance programs and practices with many programs undergcing major changes, and 3) Both the NRC and industry, through NUMARC Working Group 4, have coordinated their activities more effectively and have achieved a better understanding of maintenance issues within the last i year. NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS '

REPORT FOR JULY 28 - AUGUST 3, 1986

1. PROPOSAL TO DISPOSE ACCIDENT GENERATED WATER The licensee (GPUN) has submitted its proposal for disposing of approximately 2.1 million gallons of water that has been contaminated as a result of the TMI-2 accident and the subsequent cleanup. The proposal requests NRC approval of the licensee's plan to force evaporate the water, including Tritium (approx.1000 C1), on the TMI site over a period of about two and one-half years. The resulting evaporator bottoms, including Cesium-137 (approx. 0.3 Ci), Strontium-90 (approx 0.9 Ci),

boric acid (approx. 150 tons), and sodium hydroxide (approx. 11 tons),

would be solidified and disposed of as low-level radioactive waste in accordance with applicable regulations.

In addition to requesting NRC approval of this proposal, the licensee is -

... support of the NRC in obtaining favorable DOE consideration of the additional allocation...".

In accordance with the Commission's Statement of Policy issued April 28, 1981, which provided for Commission review and approval of this issue, the NRC staff will provide an evaluation and a recommendation on the licensee's proposal for Commission consideration.

2. DEFUELING

- The licensee completed drilling, using solid faced bits, a series of holes in the hard crust layer of the damaged core. These perforations were made so that other defueling tools can be more effectively used in breaking up and removing the debris. Attempts at penetrating the crust were made at 48 locations. Of these, 28 resulted in penetrations ranging from a depth of 12 inches to 48 inches. The remaining 20 penetrations had depths less than 12 inches. The drilling rig is being disassembled and preparations are being made to commence bulk defueling.

- The major engineering, design, procurement, and installation tasks for the TMI-2 defueling are essentially complete. Projects remaining are operational in nature. In response to these changes the licensee has reorganized and consolidated some departments.

This plan places various sections of the Recovery Programs Department into the Defueling and Site Operations Departments and abolishes the Recovery Programs Department.

3. PLANT STATUS

- The reactor remains in long term cold shutdown, vented to the atmosphere. Core cooling is by natural heat loss to ambient building atmosphere. The average incore thermocouple reading is 80*F.

- The airborne radioactivity on the defueling platform is about 8.8 E-7 uCi/cc Tritium and 3.3 E-11 uCi/cc particulates, predominately Cesium-137 and Strontium-90. The platform is mounted above the modified internals indexing fixture which is mounted over the reactor vessel. These provide about 15 feet of water over the core region and 6 feet over the carousel holding the defueling canisters.

4. WASTE MANAGEMENT

- The Submerged Demineralizer System (SDS) and EPICOR II remained in temporary shutdown this week.

- Total volume processed through SDS to date is 4,084,856 gallons, and -

the total volume processed EPICOR II to date is 3,055,238 gallons.

5. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and The City of Lancaster Agreement.

- - The Lancaster water sample taken at the water works river intake and analyzed by EPA consisted of a seven day composite sample taken July 13 - 19, 1986. A gamma scan detected no activity that could be attributed to TMI-2.

- TMI water samples taken by EPA at the plant discharge (includes Units 1 and 2) to the river consisted of seven daily composite

, samples taken from July 12 - 19, 1986. A gamma scan detected no activity that could be attributed to TMI-2.

- The EPA analysis of the NRC outdoor air sample for the period July 24 - August 1, 1986 showed that concentrations of Cs-137 and

. I-131 were below the lower limit of quantitative detectability for the system.

Status: DOE issued the final PDS on April 3 ,1986.

Action: NRC staff is currently evaluating the final PDS for DOE's response to NRC comments on the draft PDS.

Section: 114(e)(2) of NWPA: Federal Agency Reporting Requirements Status: With the issuance of the DOE final PDS on April 3, 1986, the reporting requirements of Section 114(e)(2) are now effective. Any .

Federal agency that determines that it cannot comply with any deadline in the final PDS, or fails to so comply, is required to submit a report to the Secretary of Energy and' to Congress explaining the reason for such actual or potential noncompliance.

Action: NRC staff is currently evaluating the impact of this requirement on NRC's n ar-term deadlines in the final PDS.

Section: 141 of NWPA: Monitored Retrievable Storage (MRS)

Status: DOE had planned on submitting the kRS proposal and NRC comments to Congress in early February,1986. Hovever, an injunction on behalf of the State of Tennessee has delayed the submittal of the proposal indefinitely.

Action: DOE has filed an appeal to expedite a decision on the injunction in the Sixth Circuit Court of Appeals in Cincinnati, Ohio, and oral arguments were held on July 24, 1986. A decision is expected in 60-90 days.

Section: 306 of NWPA: Regulations for Training Power Plant Personnel Status: The final rulemaking on Part 55 and three associated regulatory guides concerning regulations for Training of Power Plant personnel was submitted to the Commission on April 18, 1986 (SECY-86-123) and is currently awaiting Commission approval, pending OGC analysis of the package.

. 2 Near-Term DOE Actions of Interest August, 1986 - Issue final Transportation Insitutional Plan -

Application of International Safeguards at Exxon Although the Exxon LEU fuel fabrication facility at Richland, Washington is no longer under IAEA inspection, agreement has been reached between the IAEA, the Department of State, the NRC, the Taiwan Power Authority and Exxon to allow an IAEA inspector to visit the Exxon facility.

The IAEA inspector has permission to randomly select fuel assemblies for verification of nuclear content and attach IAEA seals to 112 shipping crates containing fuel bound for Taiwan. The seals will be removed by the IAEA when the fuel is loaded into a reactor.

This procedure allows the IAEA to meet a safeguards timeliness goal.

Safeguards Regulatory Effectiveness Review (RER) at Yankee-Rowe l

The NMSS Safeguards staff, assisted by U.S. Army Special Forces personnel, conducted an RER at the Yankee-Rowe nuclear power plant from July 28 to August 1, 1986.

West Valley Snipment Study FC has completed its review of the draft final report entitled " Case Histories of West Valley Spent Fuel Shipments" submitted by Aerospace Corporation.

Staff has forwarded a number of comments to the contractor, suggesting a redraft of the report. Staff anticipates completion of the final report in August 1986.

Nuclear Fuel Services Inc.

On July 24, 1986, members of NMSS, SP, OGC, and IE met with Mr. Patrick Stevens, Nuclad, Inc., Denver, Colorado, to discuss what information would be needed for NRC approval to transfer ownership of a licensed facility. Nuclad, Inc., is interested in purchasing the NFS, Erwin facility.

Status of Hearings

1. Sequoyah Fuels, Gore, Oklahoma, UF6 to UF4 Administrative Judge Frye has scheduled a meeting of all parties to the informal hearing on August 1,1986, in Sallisaw, Oklahoma. The informal hearing has now been tentatively rescheduled for the week of August 11, 1986. However, by memorandum dated July 3,1986, Judge Frye recommended to the Commission the adoption of formal hearing procedures. Action by the Commission on these recommendations may impact the schedule for the hearing.
2. Sequoyah Fuels, Gore, Oklahoma, Comprehensive Waste Disposal Plan The hearing is being held in abeyance pending submittal of a new plan as part of the revision to the decommissioning plan required by Source Material License No. SUB-1010, Condition No. 21.

'3. Kerr-McGee, West Chicago, Illinois, Kress Creek Decontamination The ASLB decision dated June 19, 1986, dismissed the Order to Show Cause issued to Kerr-McGee Chemical Corporation on March 2, 1984. On June 30, 1986, the staff filed a notice of appeal with ASLAB.

4. Kerr-McGee Rare Earths Facility, Decommissioning Plan, West Chicago, Illinois This hearing is being held in abeyance pending the staff's completion of the supplement to the Environmental Impact Statement. The current schedule calls for issuance of the Draft Supplement by October 1, 1986, and the Final by May 1, 1987. The Board has indicated that they may resume a portion of the Hearing after the Draft is issued.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending August 1, 1986 American Statistical Society Conference on Radiation and Health.

During this conference, held July 20, 1986, the latest information on the reassessment of radiation doses received by atomic bomb survivors in Hiroshima and Nagasaki (SECY-86-95; 3/24/86) was presented. The new dosimetry system (DS-86) has been installed in the computer of the Radiation Effects Research Foundation (RERF), and preliminary outputs indicate that the low-LET risk coefficients for leukemia and other cancers will increase y about a factor of two. A major perturbation introduced by DS-86 is a much smuller dose due to neutrons at Hiroshima than was previously estimated. As a result, it appears ,

that it will be impossible to determine the relative (to low LET radiation) effectiveness (in cancer inductions) of neutron radiation based on the Hiroshima and Nagasaki experience. The value selected for this effectiveness (which will have to be based on experimental work) will have considerable impact on low-LET risk coefficients. The Committee on Interagency Radiation Research and Policy Coordination (CIRRPC) is planning to fund a study of this and other radiation risk questions by the National Academy of Sciences (BEIR V Committee).

Preliminary reports at the Conference on several epidemiological studies (not sponsored by the NRC) of nuclear power plant workers in the U.S. and Canada were presented. Few conclusions can be drawn at this time. The conference participants, however, expressed their disappointment that the dose reporting requirements in the proposed revision to 10 CFR Part 20 do not improve the current status; i.e., determination of individual doses can be made only from termination reports but not from annual reports supplied to the NRC by the licensees. Such information is required for epidemiological studies, and the participants were urged by the speakers to send appropriate comments to the NRC.

RES RULEMAKING ACTIVITIES Emergency Core Cooling Systems; Revisions to Acceptance Criteria (Part 50)

(Appendix K)

The proposed rule would amend regulations concerning acceptance criteria for emergency core cooling systems (ECCS) by changing the methods used to demonstrate that an ECCS would protect the nuclear reactor core during a loss-of-coolant accident. Because the proposed rule represents a significant change in a regulatory requirement, the staff is currently preparing a summary of ECCS research performed over the last 10 years which will serve as the technical basis for the proposed rule and a regulatory guide which will provide definition of what constitutes an acceptable best estimate model and acceptable .

A meeting was held with the CRGR on July 23, 1986, to discuss the Commission paper, the proposed rulemaking, and the regulatory guide. The CRGR approved the rulemaking subject to comments provided by the CRGR at the meeting. The staff is to return to the CRGR on August 13, 1986, with a revised section on reporting requirements. A statement is also to be submitted by the Acting Director, RES, that provides his evaluation of the safety and cost impacts of the proposed rulemaking.

Standards for Protection Against Radiation (Part 20)

Radiation protection philosophy and technology have changed markedly since the present Part 20 was promulgated nearly 30 years ago. A complete revision is needed to reflect current information on radiation risk, dosimetry, and protection practices; to be consistent with recommendations of international authorities (ICRP); and to comply with anticipated Presidential guidance to Federal agencies as recently developed under EPA leadership.

A draft backfit analysis has been approved by the Commission for publication and will be forwarded to the Office of the Federal Register for publication for public comment. The comment period on the proposed revision to 10 CFR Part 20 will be further extended to provide a joint 60-day comment period on both the draft backfit analysis and the Part 20 proposed revision.

This was probably GRECA's last meeting as the work is virtually completed.

Court Injunction The Commission, by a 3-1 vote, approved an order responding to a request for a stay.of the immediate effectiveness of two license

amendments issued by the NRC staff on May 30, 1986. The amend-l ments authorized the Pacific Gas and Electric Company to rerack i the spent fuel pools at~both units of its Diablo Canyon Nuclear i Power Plant.
The order stayed, pending completion of the on-going license amendment hearing and initial decision, that portion of the

. amendments which authorized PG&E to store in excess of 270 fuel assemblies in either of the two pools at Diablo Canyon. In all other respects the order denied the petitioners request for a stay. Commissioner Asselstine disapproved and provided separate views. Chairman Zech and Commissioners Roberts and Bernthal also provided additional views in response to Commissioner Asselstine.

(Subsequently, on July 22, 1986 the Secretary signed the order.)
