NOTICE OF VIOLATION AND NRC. INSPECTION H PORT NO. 50-302/97-14-
Dear Mr. Anderson:
An NRC inspection was conducted October 6 - 10. and October 20 - 24, 1997, at
,
.
your Crystal River facility.- This was a Safety System Functional Inspection
,
(SSFI) of the makeup and purification system and decay heat removal system.
The enclosed report presents the results of that inspection.
,
Overall, the operational performance capability of the mdeup and purification
system and the decay heat removal system was assessed as adequate with some
.
-exceptions. - The-exceptions were discussed in th'. exit meetings and some
.
'
involved submittal of several requests for assistance to the NRC Office of
,
-Nuclear Reactor Regulation for disposition.
The SSFI team noted that-
'
significant work involving plant modification and revision of documentation
(procedures'and calculations) wn ongoing during the inspection period.
This-
,
work limited the SSFI team's capability to evaluate the.aperational. readiness
i-
of the two focus systems fully. ~ The SSFI team identified several conditions.
'
in which ongoing corrective-actions for past problems did not allow for
,
' assessment-of readiness to_ resume operations. Each of these items ~was-
'
. identified in this report and linked to an open NRC' issue or a new NRC issue

was -identified. -In addition dua to the recent changes in the corrective

action program, the team was unable to evaluate the effectiveness in this

area,

Based on the results'of the inspection, the NRC has determined that violations
j
of NRC requirements occurred.
Violations were identified-for failure to take
e
adequate corrective actions in the areas of stem protectors for valve
actuators and drawing deficiencies, failure to test makeup valves adequately,
an inadequate procedure for calibration of reduced inventory operation level
!
instrumentation, failure.to evaluate the effects of out-of-tolerance test
equipment, failure to follow foreign material exclusion procedure
requirements, failure to install overload relays properly, and failure to take
!
adequate corrective actions for past design _ weaknesses. Although the number
of violations identified indicated additional need for management attention in
_

assuring regulatory requirements were met, the NRC considered these issues
l
were isolated instances of i gulatory non-compliance.
