FERMI 2 SOUTH REACTOR FEEDWATER PUMP TURBINE FAILURE (DN50-341)
Enclosed is the final inspection report feeder prepared by E. L. Meils, a Region III consultant.
I am of the view that this report is sufficient to address the specific feedwater turbine allegations that are part of our overall action plan containing this and several other allegations.
I recomend you include applicable documentation on this issue in one special inspection report which I envision would address all of the outstanding Fermi allegations.
After reviewing the report contents, your final write-up should include an evaluation for enforcement action covering the following areas:
1.
Deficient installation documentation 2.
Turbine to pump alignment not performed according to procedure 3.
Failure to incorporate warm-up procedures during the preoperational test program.
4.
Failure of operations to investigate vibration alarms 5.
Adequacy o' the preoperational test program.
For information, I still plan to meet with the alleger and go over the action plan we have developed.
I would anticipate completing this effort in early March.
In the interim, please provide me with a status report on allegation followup and any items which would require additional inspection prior to a plant restart.
