I.
PURPOSE The purpose of the surveillance was to verify compliance to the administrative requirements of EBASCO Company Procedure No. E-30, "Preparation of Calculations" dated February 20, 1983.
II.
SCOPE The surveillance consisted of a review of EBASCO calculations AS 001, l
l Revision 0, "Weld Interaction Ratios - Comparison of As Built VS l
As-Designed Welds;" AS-002, Revision 2. "Seal Welds in AB DWGS-Assumptions l
to be Made in DV;" and AS-003, Revision 3, "Base Metal Defects / Assumptions to be Made for Hidden Members."
III.
SUMMARY
The surveillance identified five (5) deficiencies, three (3) discrepancies, and eight (8) observations in which the calculations do not administrative 1y comply with the procedure requirements. The deficiencies identified instances of improper revision of calc.ulations, originator and checker being the same individual, unauthorized calculation forms used, checking functions not documented, approvad signature not provided and calculation number not consistent with procedural requirements. The technical adequacy of the calculations was. not verified.

FINDINCS DEFICIENCIES Deficiency No. 1 Section 6.1 states in part:
"Hand calculations are prepared on standard calculation sheets (Form 581)."
Contrary to this requirement, calculation AS-001 (Pages 4 and 4A), AS-003 (Pages 10 and 52 thru 64) were not 581 forms (i.e. TUCCO Calculation Sheets).
Deficiency No. 2 Section 6.1(d) and 6.2.1 state that the "Originator and checker shall sign the first sheet of the calculation package."
Contrary to this requirement, the Originator.and; checker for calculation.,
s packages AS-001 and AS 003 failed to sign the first sheet.
Deficiency No.' 3 Section 6.2.1 states:
"The checker shall initial and date each page of the original calculations after they are completely checked and all necessary corrections and additions have been made and then sign the first page of the calculation."
Contrary to the above, all three calculation packages failed to meet this requirement.
Deficiency No. 4 Section 6.2.1 states:
"The checker shall initial and date each page and sign the cover page."
Contrary to this requirement, calculation package AS 002 Page 18 of 23 (replacement) was originated by A. Salia on 9/15/87 but cover page identifies A.Salim also as the checker on 9/15/87.

NE-15917 Deficiency No. 5 Section 7.2 states:
"All changes shall be identified with a revision
-number on the calculation sheet."
Section 7.4 states in part:
"Changes are initialed and dated as for original calculations.
The change shall be identified with a new revision number on the calculation sheet. The first sheet of the calculation package shall indicate the new revision."
Contrary to the above requirements, all three calculation packages contained violations of these change requirements.
For example, AS-001 pages 3, 3A, 4A, 29; AS-002 pages 1, 3, 4, 5, 10; and AS 003 page 5 were revised with no change to calculation cover pages.
DR RESPONSIBILITY:
EBASCO DISCREPANCIES Discrepancy No. 1 Section'5.4 s'ates: '"Assumptions and other dat'a shall be clearly' stated,'
t wl. ore they are introduced into the calculations, with justifications and source (other calculations, text books, tables, standards)."
The procedure does not address the methodology of incorporating the above mentioned items into the calculation.
It does not mention the attachments or appendicos that are also found in all of the calculation packages reviewed.
Instructions should include pagination, title, table of contents entry, etc. for each attachment / appendix / table.
Discrepancy No. 2 The process of identifying total pages of a calculation is not proceduralized.
Presently, the calculation packages have errors in pagination. Pages are added but not identified on cover page (i.e., AS-001 pages 3A, 4A, 14A; AS-002, number of sheets specified as 24 - actually 29 pages of calculation; AS 003 identifies 64 pages of calculation with 3 appendices actually the calculation has 64 pages, 3 attachments and 1 appendix (not listed).

NE-15917 Discrepancy No. 3 Section 6.3 states: "Verification of calculations will be in accordance with Engineering Procedure E-76, documentation of verification will be shown in paragraph 6.2.1."
Section 6.2.1 states: "The checker shall initial and date each page of the original calculations after they are completely checked and all necessary corrections and additions have been made and then sign the first page of the calculation."
The procedure requirements are conflicting. Procedure E-76 requires a form 0599 to be used per Attachment 5 for documenting verification while procedure E-30 requires the documenting of verification per Section 6.2.1.

Action Responsibility : EBASCO OBSERVATIONS:
Eight observations were made on the three calculation as shown on.
s Action Responsibility : EBASCO No response is required for deficiencies identified in this report as corrective action is addressed on DR No.C-88-00073. A written response to the discrepancy and the nine observations is to be submitted to EA Evaluations by February 11, 1988. The response should include the cause of the findings reported, extent of conditions, action to resolve the reported condition, action to prevent recurrence and scheduled action completion dates.

NE-15917 ATTACHMENT 1 OBSERVATIONS CALCULATION AS-001 1.
Section titled "Problem" on cover page, last sentence states: "When final, further confirmation of the corrections of this comparison needs to be made."
However, the section "Assumptions confirmed " is blank and the cover page is marked as being "final."
CALCULATION AS-002 1.
Table 3, page 22 is not listed in the Table of Contents (Tables 1 and 2 are listed).

The Subject on page 2 of 23 is different than the cover page.
3.
Calculation pages have been marked "deleted" by an engineer whereas they a
were marked "superseded" by another engineer.

Document in calculation package titled "Data Check & Verification of-Calculation AS-002" is not listed on Table of Contents.
CALCULATION AS-003 1.
Section 5.1.3 states in part:
"When an originator assigns the calculation number, the number shall be prefixed by the originator's initials."
All three calculation numbers have an "AS" prefix, but the originator of AS-001 is P. Winkler, AS 002 and AS-003 is R. C. Iotti.

Section "Preliminary" or "Final" of the cover page is not marked.
