December 18, 1987 To: Distribution COMANCHE PEAK STEAM ELECTRIC STATION ENGINEERING ASSURANCE SURVEILIANCE REPORT TSR 87-84 DEFICIENCY REPORTING PROCEDURES A surveillance of lead engineering contractor's deficiency reporting procedures was conducted from December 8, 1987 through December 9, 1987 by Comanche Peak Engineering - Engineering Assurance 4CPE-EA).
I.
PURPOSE The purpose of the surveillance was to verify that Ebasco, Impell, Stone and Webster Engineering Corporation - Corrective Action Program (SWEC-CAP),
and Stone and Webster Engineering Corporation - Pipe Support Engineering (SWEC-PSE), both on site and off site, have procedures that addrese the documentation of design deficfencies in a manner that is compatible with ECE 3.06, "Reporting and Control of Deficiencies."
II.
SCOPE The scope of the surveillance involved review of seven procedures with regard to documenting design deficiencies.
Procedures reviewed and the associated lead engineering contractor grouar are listed in Attachment 1.
III.
SUMMARY
A review of procedures CPSP-20, IMT-FP-35, and IMT-EQ-22 indicated no problems with regard to procedural requirements for documenting design deficiencies. A review of the remaining procedures resulted in identifying one (1) deficiency and three (3) discrepancies.
IV.
FINDINGS DEFICIENCY 1.
Although Impell procedures PI-15 and 0210-053 008 address review and disposition of Deficiency Reports, the procedures do not address the 1.nitiation of Deficiency Reports for dasign deficiencies as required by ECE 3.06.
Deficiency Report C-87-05063 has been initiated regarding this problem.

DISCREPANCIES Discrepancy No. 1 Although Procedure PP-059 addresses the initiation of Deficiency Reports, it does not specifically address the initiation of DRs for design deficiencies.
Discussions with SWEC-CAP indicate that an internal SWEC document, i.e.,
Report of a Problem (ROAP), is used to document design deficiencies. As a result, it is requested that SWEC-CAP review and clarify, as necessary, PP-059 so as to be compatible with the requirements of ECE 3.06 with regard to initiating Deficiency Reports for design deficiet cies.
Action Res onsibility : SWEC-CAP Discrepancy No. 2 Discussions with SWEC PSE indicate that offsite SWEC groups which are performing su. pott activities for SWEC-PSE use ROAPs for reporting design deficiencies.
'As a' result, it is requested that SWEC-PSE initiate action to review and clarify procedures, as necessary, such that offsite SWEC groups which are performing support activities for SWEC PSE initiate Deficiency Reports for design deficiencies in a manner that is compatible with ECE 3.06.
Action Responsibility : SWEC-PSE Discrepancy No. 3 Discussions with Ebasco indicate that offsite Ebasco support groups use nonconformance reports, in accordance with the Ebasco QA Program, to document design deficiencies.
As a result, it is requested that Ebasco initiate action to review and clarify procedures, as necessary, such that offsite Ebasco groups which are performing support activities initiate Deficiency Reports for design deficiencies in a manner that is compatible with ECE 3.06.
Action Responsibility : Ebasco OBSERVATIONS: None The findings 'eri discussed via telecon with Suzzane Crozanick (Impell), Peter Hunt (SWEC CAP), Carl Aldridge (SWEC-PSE), Joe Dwyer (Ebasco) and Herman Kunis (Ebasco) during the surveillance. As the cited deficiency regarding Impell procedures has been documented on a Deficiency Report, no additional response to the deficiency is required. However, written responses to the three (3) discrepancies by the indicated affected contractors are required to be submitted not later than January 25, 1988.
