APPENDIX A NOTICE OF VIOLATION Florida Power Corporation license No.: DPR-72 Based on the results of the NRC inspection conducted on August 30 -

September 1, 1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below.

These items have been categorized as described in our correspondence to you dated December 31, 1974.

Technical Specification 6.8.1 requires that, written administrative procedures concerning procedure review and approval shall be established, implemented and maintained for those safety-ralated activities listed in Appendix A to Regulatory Guide 1.33 (1972). Section 2.7 (Procedures) of Administrative Instruction AI-500 concerning conduct of operations states in part that procedures requiring verification will be handled as follows; When the operator has performed an operation or verified the status of the item as required ... he initials the space provided for that step in the procedure ... the completed procedure is then reviewed by the Shift Supervisor for completion ... W en his review is complete the Shift Supervisor signs in the approved space.

Contrary to the above, on August 31, 1978 it was found that procedure signoffs were made in Paragraph 6.1.2 of OP-202 " Plant Heatup Procedure," indicating that certain subsystems were filled, vented, aligned, and in the mode required for plant startup when the referenced subsystem procedures had not been completed as required in that:

1. Steps 7.1, 7.6 and 7.7 of the startup section of OP-402, " Makeup and Purification System Operating Procedure," as well as the Shift Supervisor review had not been completed as required.

. Additionally, approximately ten valves operated from and indicated in the control room had not had their position established and/or verified as required by the procedure.

2. Step 6.1.1 of OP-405, " Reactor Building Spray System Operating Procedure," had been signed indicating satisfactory completion of the initial valve lineup when valve position verifications had not been completed and the Shift Supervisor review signatures.for Section 6.1 (Conditions for Modes 1, 3 and 4) and Section 6.2 (Conditions for Modes 5 and 6) were missing.
3. The shift supervisor did not indicate his review of the initial condition, filling and venting, and startup section of OP-404 and OP-408, Operating Procedures for the Decay Heat Removal System and the Nuclear Services Cooling System, respectively.

This ites of noncompliance is an infraction.
