CRYSTAL RIVER UNIT 3 - REVIEW OF INDIVIDUAL PLANT EXAMINATION SUBMITTAL - INTERNAL EVENTS (TAC NO. M74401) i

Dear Mr. Anderson:

On March 9,1993, you submitted the Crystal River Unit 3 (CR3) Individual Plant Evaluation (IPE), in response to Generic Letter (GL) 88-20 and associated supplements.

By letter dated November 22, 1995, you provided responses to our request for additional information (RAI).

The staff, with its consultant, Brookhaven National Laboratory (the contractor), also had teleconferences with you in July 1996 to discuss several technical aspects of your submittals.

With assistance from our contractor, we have reviewed your submittals.

Based on our review, we find that your submittals do not include sufficient information for us to conclude that you have met the intent of the GL. The enclosed safety evaluation (SE) documents the basis for our conclusion.

The contractor's technical evaluation report is attached as an appendix to our SE.

Please be advised that as part of our quality assurance process, our SE was reviewed by an IPE Senior Review Board comprised of staff and consultants at Sandia National Laboratory with probablistic risk analysis (PRA) expertise.

Our SE lists areas of concern in all three primary review areas; front-end, back-end, and human reliability analysis.

CR3 has the lowest reported core damage frequency (CDF) of the Babcox & Wilcox plants.

However, the low CDF in of itself, is not necessarily indicative of an adequate PRA process.

It is important to address the weaknesses identified in our SE to understand their role and contribution to the CDF and the overall IPE process.

We are forwarding our SE to you so that you may review the identified areas of weaknesses and take appropriate actions.
