On 9-11-85 at 2330, with the unit subcritical, it was discovered that the Average Power Range 5fonitor (APRSI) weekly Reactor Protection System (RPS) functional tests (Channels B, D, E, G,11) did not satisfy the requirements for the setdown s

i neutron trip. During this time, core alterations had been performed with less than the minimum number of APR>f channels.

The Shif t Supervisor declared the above mentioned APR5fs inoperable.

The required APR31 weekly RPS functional tests were completed, and core alterations j

resumed.

._vco nu,,on When the APRM weekly RPS functional tests were performed on 9-3-85 (Channel F) and 9-6-85 (Channels B, D, E, G,11), steps not required in Mode 5 were not performed to reduce unnecessary testing. On 9-11-85 the procedure results were reviewed by the Technical Staff System Engineer. During this review it was found that the Mode 5 test requirements for the setdown neutron trip were not satisfied. Normally when this trip is checked a half scram is verified, however, in this case the steps to verify the half scram were left out.

The Shift Supervisor concurred with the finding and declared the above APRMs inoperable. Since 9-6-85 core alterations had been performed with less than the minimum number of APRM channels.

As a result the limiting conditions for operation for Technical Specification 3/4.3.1 were not satisfied.

The requirements of Section 3.0.4 were also not satisfied.

No core alterations were being performed when this was discovered.

The procedures were then performed with all the steps required for Mode 5.

Core alterations resumed when the required APRM weekly functional tests were completed.
