AFFIDAVIT OF DAVID H. WADE THE STATE OF TEXAS COUNTY OF DALLAS BEFORE ME, the undersigned authority, a Notary Public in and for Dallas County, Texas, on this day personally appeared DAVID H. WADE, who by me being duly sworn upon his oath depose,s and says:
My name is David H. Wade. I am presently employed by Texas Utilities Generating Company as a Mechanical Engineering Discipline Supervisor in Opera-tions Support Engineering in Dallas, Texas. I reside at 5225 Ponderosa Way, Dallas, Texas 75227. My telephone numbers are 214/979-8371 and 214/388-1315. In 1978, I was an Engineering Supervisor at the Comanche Peak Steam Electric Station near Glen Rose, Texas. In 1980, I was a Mechanical Engineering Discipline Supervisor at Comanche Peak. On December 31,1984,'I was an Administrative Assistant to the Project General Manager at Comanche Peak.
I was not interviewed by Management Analysis Company (MAC) in 1978 in connection with any quality assurance review of Comanche Peak. I did not attend any initial interview regarding the MAC Report nor did I attend pre-or post-audit meetings. I did not participate in any internal management discussions regarding the report and was not aware that there was a report that should have been provided to CASE on discovery. I first learned that MAC had issued a report relating to the QA program at Comanche Peak when I read about it in the newspaper in May or June of 1985. I do not have a copy of the report, have never had a copy of the report, and have not read the report.
