SUBJECT:
CATAWBA NUCLEAR STATION: DISPOSITION OF R2A'S On March 21, 1984, during the interview of a welding inspector, currently employed by Duke Power Company (DPC) at the Catawba Nuclear Station, it was learned that he had some concern over the final resolution / disposition of two separate R2A's (Construction Deficiency Reports), that he had written some months ago.
The first concern was on an I-beam he was inspecting in the reactor building (elevation 525, 125 ).
He said he saw a crack emanating from a weld approximately two feet to the end of the I-beam.
He said he wrote R2A Number W205, which was later upgraded to NCI Number 16672.
He said the resolution was to grind out the weld and the cracked area, and fill it with filler material.
He said his concern was how far thh crack went behind the base plate, and it did not appear that DPC tried to ascertain the extent of the problem.
Additionally, the interviewee related that while inspecting instrumentation piping in the reactor building, he observed what appeared to be burn-through from the vendor supplied piping.
He said he was told that vendor supplied piping was not addressed in the ASME piping code so the burn-through was not considered to be a problem.
He provided three R2A's that he had written on the problems, R2A Numbers W370, W372 and W373.
At the conclusion of the interview it was determined that someone, preferably the resident inspector, address these issues to determine if potential problems exist.
This information is provided to you for action deemed appropriate.
We would appreciate a written response regarding the findings if action is taken.
