This inspection was performed using the guidance of NRC Inspection Procedure 73753,

" Inservice inspection," dated May 4,1995, to determine whether the inservice inspection, repair, and replacement of Class 1,2, and 3 pressure retaining components at Arkansas Nuclear One, Unit 1, have been performed in accordance with Technical Specifications, the applicable ASME Code, correspondence between NRC and the licensee concerning relief requests, and requirements imposed by NRC/ industry initiatives. Additionally, a previously identified violation on Unit 2 was close Maintenance

  • The job orders, work requests, weld procedure specifications, and general welding standard were well developed (Section M3).
  • The observed welding activities were performed well and in accordance with the appropriate procedures (Section M4).

i The inspector reviewed the documents listed in the attachment for the examination ,

of one weld, the replacement of two components, and the repair and replacement of  ;

i a core flood tank drain valve and line. The inspector reviewed these documents to determine if they had been developed in accordance with regulatory requirements and the applicable ASME code requirements,

! Observations and Findinas The inspector observed that the weld procedure specifications clearly identified the ,

material to be used in performing the welds. The inspector noted that the weld procedure specifications also identified the applicable variables to be monitored during the welding process. The inspector also observed that the job orders and work requests were clearly written, comprehensive, and easy to follow. The inspector found that the documents reviewed had been developed in accordance with licensee procedures, regulatory requirements, and applicable ASME code requirement Conclusions The inspector concluded that the procedures and documents were well developed for the identified task M4 Maintenance Staff Knowledge and Performance Inspection Scope (73753)

The inspector witnessed welding activities on three joints, a liquid penetrant examination on a fourth weld, and the shop fabrication of a replacemerit valve and its associated piping. The inspector also reviewed the weld procedure specifications listed in the attachment.

l b. Observations and Findinas l The inspector observed a licensee contractor welder perform the welding on Field Weld 29AC1 EA55, downstream of an atmospheric dump valve. The inspector found that the welder performed the weld in accordance with the applicable weld ;

procedure specificatio l The inspector observed two semi-automatic welds performed by licensee contractor welders. One weld was on Field Weld 114, downstream of a main feedwater flow venturi, and the other was on Nozzle N-1 of the E1B feedwater heater. The inspector found that these welds were performed in accordance with the applicable weld procedure specification The inspector also observed a licensee welder perform the f abrication of a replacement valve and its associated piping in the welding shop. The inspector found that the welder performed the tasks in accordance with the applicable weld procedure specification and the associated job orde The inspector observed a licensee contractor nondestructive examination person properly perform the liquid penetrant examination on Field Weld 25-036 on Valve RBD-9 c. Conclusions The inspector concluded that the observed welding activities were performed well and in accordance with the appropriate procedures. The inspector also concluded that the observed nondestructive examination was performed in accordance with the appropriate procedure M8 Miscellaneous Maintenance issues (92902)

(Closed) Violation 368/9523-01: Failure to Verify interpass Temperature After Root l Pas This violation was identified during an inspection of the licensee's inservice inspection activities, during which welders did not measure interpass temperatures after completing the root pass as required by procedur The inspector noted that the licensee acknowledged this violanon by letter dated November 21,1995. The inspector found that the licensee had completed the actions as described in the letter. The inspector also found that, subsequent to the completion of the described corrective actions, the licensee implemented a company-wide general welding specification, E-GWS-1, " General Welding Standard -

ASME/ ANSI /APl/AWWA," effective on April 29,1996, that superseded Specification APL-M-2415, " Nuclear Welding Standards for Arkansas Nuclear One -

5-Units 1 & 2," Revision 6. The inspector verified that the new standard contained appropriate guidance for the measurement of interpass temperature Ill. Enoineerina E2 Engineering Support of Facilities and Equipment E2.3 Review of Safety Analysis Reoort Commitments A recent discovery of a licensee operating their facility in a manner contrary to their Safety Analysis Report description highlighted the need for a special focused review that compares plant practices, procedures, and/or parameters to the Safety Analysis Report descriptions. While performing the inspection discussed in this report, the inspector reviewed Sections 4.2.5.1, " Decay Heat Removal"; 4.4, " Tests and Inspections"; and 6.1.2.1.3," Core Flood System," of the licensee's Unit 1 Safety Analysis Report. The inspector did not identify any discrepancies between the Safety Analysis Report and the inservice inspection program or the systems reviewe E3 Engineering Procedures and Documentation Scone (73753)

The inspector observed that the change package was prepared in accordance with regulatory requirements and licensee procedures.
