RESPONSE TO NOTICE OF VIOLATION (NRC Inspection Report Nos. 50-313/96-21 and 50-368/96-21)
Dear Mr. Hutchinson:
This will acknowledge receipt of your letter dated October 7,1996, and your check for $50,000 in payment for the civil penalty proposed by NRC in a letter and Notice of Violation and Imposition of Civil Penalty (Notice) dated i
September 6, 1996.

We find your reply generally responsive to our concerns and your corrective actions will be reviewed during a future inspection. The NRC does note that i
in the section of the response to the Notice that addressees your admission or
denial of the violations, you admit to failing to properly install the cotter pin in the release nut for valve PSV-2685 at AN0-1.
While this response neither admits nor denies the violations, we acknowledge that the remainder of your letter is responsive to the root causes of the violations cited in the

Notice, and the corrective actions you have proposed appear to adequately address these root causes in order to preclude recurrence.
For these reasons,

in addition to the fact you have paid the civil penalty, NRC presumes you admit the violations occurred as described in the Notice.
