Paga1 of 2 SECTION 1 INTRODUCTION AND SCOPE This Summary Report covers the Inservice inspection performed at Clinton Power Station (CPS), between December 10,1993 and April 29,1995. This is the first inspection performed during the third inspection period of the first ten (10)-year interval. During this Fuel Cycle, CPS experienced 2 planned outages and 6 forced outages. These planned outages were PO-5 and PO-6 and forced outages were FO-93-01, FO-94-01 A, FO-94-01B, MO-5, MO-6, and FO-95-01 for repairs. All examinations, tests, repairs and replacements were performed in accordance with the CPS ISI Program Manual which complies with ASME Section XI,1980 Edition through Winter 1981 Addenda.

The ASME Section XI examinations, tests and repairs / replacements were witnessed or venfied by Authorized Nuclear Inservice Inspectors W. Ruggles, R. Weiss, R. Riegler, and D. Mcdonald of Arkwright Mutual insurance Company (Factory Mutual Engineering Association). This summary report contains additional augmented (non-ASME Section XI) inspection results not required to be witnessed or venfied by the ANil.

In accordance with IWA-6220, the name and description of the components examined are listed on the NIS-1 Data Report and its attachments (see Section 2). Further component information is obtainable through the reference number, or other information, for the specific item. The manufacturers' names for the components examined are also identified in item 7 of the NIS-1 Data Report and their addresses are on file at CPS.

Documentation supporting this Summary Report is referenced in the Inservice Examination Plan, implementing p procedures, or record files identified in the specific sections of this report.

Scope:

The scope of inservice inspection performed during this Fuel Cycle is summarized as follows:

l

• Preservice Examinations - Preservice examinations were performed on bolting associated with the main steam safety relief valves, feedwater check valves, and on various supports added to replace snubbers.

• Jet Pump Hold down Beams -In RF-5, Illinois Power replaced 19 of 20 jet pump hold down beams with materials having an improved heat treatment. A liquid penetrant test and ultrasonic examination was performed to ensure that there were no manufacturing flaws and to document the existing condition of beams. One jet pump hold down beam was replaced during RF-4, with the improved heat treated material.

• Scheduled inservice Examinations - The NIS-1 Data Report (Section 2) provides the listing of all examinations completed during RF-5. Conditions noted during RF-5 and corrective measures taken are described in the NIS-1 Data Report.

• Snubbers - The RF-4 Summary Report identified the number of Technical Specification snubbeis as 636.

During RF-5, Plant Modifications M-071 and M-073 were implemented which removed forty-nine (49) snubbers from the piping systems. This leaves 587 Technical Specification snubbers in the ISI Program. A l representative sample of sixty-one (61) snubbers were selected for functional testing. All of these snubbers met l the functional test acceptance enteria. The three (3) snubbers which failed during RF-4 were also tested.

These snubbers were all acceptable.

• Component Supports - One hundred twenty-eight (128) component supports, other than snubbers, were I scheduled for visual examination for RF-5. One hundred four (104) supports were examined and found acceptable. Twenty-four (24) supports were deferred until RF-6.

, INTRODUCTION AND SCOPE

• Pressure Tests - ASME Code Class 1 Components - A system leakage test and VT-2 inspection of ASME Code Class 1 components was successfully performed prior to plant startup. The Nuclear Regulatory Commission has authorized Illinois Power to use ASME Code Case N-498-1, in lieu of performing the Code-required hydrostatic pressure tests for Class 3 systems.

• Erosion / Corrosion Inspections - Ultrasonic (UT) wall thickness measurements were taken at 72 locations in 7 systems during RF-5. This included 48 first time inspection locations and 24 repeat inspections. Alllocations were analyzed and found acceptable with the exception of 6 items which were replaced as a prudent measure and one other item which required replacement due to a through wallleak.

• Repairs and Replacements - No repairs or replacements were required as a result of scheduled examinations.

The Nuclear Regulatory Commission has authorized Illinois Power to use ASME Code Case N-416-1, in lieu of performing the Code-required hydrostatic pressure tests for welded repairs or installation of replacement items by welding. Modifications as well as repairs and replacements which resulted from routine maintenance are specifically detailed in Section 6 of this report.

Listing of Examinations:

Table 1 lists scheduled examinations of components for RF-5. The results column in the table provides further explanation for these examinations. Those items which were examined that require no further evaluation are identified as acceptable. Some items were deferred to later outages for various reasons and the new examination schedule for these items is identified. Some items were substituted for other items and the substituted reference number is identified. These substituted items are listed in Table 2. Also listed in Table 2 are several piping welds / items added to the RF-5 scope for various reasons and examined.

Table 3 lists scheduled usual examinations of component supports and functional tests of snubbers. The results column in the table provides further explanation for these examinations / tests. Those items which were examined / tested that require no further evaluation are identified as acceptable. Some items were deferred to later outages for various reasons and the new examination schedule for these items is identified. The visual examination results of all snubbers were acceptable. One snubber was substituted for another snubber and the substituted

( snubber number is identified. The substituted snubber is listed in Table 4.

Table 5 lists scheduled pressure tests. The results column in the table provides further explanation for these tests.

Table 6 lists scheduled erosion / corrosion examinations. The results column in the table provides further explanation for these examinations. Listed in Table 7 are several inspection locations added due to the through wall leak on line 1GS15B-6. The results column in the table provides further explanation for these examinations.

Current interval Status:

During the fifth refueling outage (RF-5),15% of the required examinations during the first ten year interval were completed. Cumulatively at the end of RF-5,94% of all required examinations have been completed in accordance with IWA-2400 (Inspection Program B) for the first ten year interval. The calculation does not include system pressure tests or snubber visual examinations and functional tests which have specific requirements for completion during specific inspection periods and refueling outages.

During the first four refueling outages (RF-1, RF-2, RF-3 and RF-4) twenty (20) items were identified as requiring further action. These items are identified as HEA-1, RHR-A-2, LPCS-2,1-RH-16-10-2, HPCS-2,1-MS-A-7PR-WA, RPV-3A, N1B, N2B, N2C, N2D, N2E, N48, N4C, N6B, N6C, N10, N16, CH-C-2 and 1-RR-A-19. There was an error in RF-4 Summary Report regarding number of items identified during RF-4 requiring further action. The correct number is 14 instead of 7. Therefore, at the end of RF-4 there were a total of twenty (20) items requiring further evaluation. These items were partially examined. In this summary report (RF-5) there are six (6) items requiring further action. Therefore, currently there are a total of twenty-six (26) items that have been partially examined and will require further evaluation prior to the end of the first ten (10)-year inspection interval.

A visual examination of the pre-existing crack in drain channel #8, weld V-16, indicated no measurable growth. The crack starts from the bottom end of the weld and extends about 7 5/8" upward. I Note 2:

The ultrasonic examination resulted in only partial coverage due to configuration and geometry. This is pending l further evaluation. The Magnetic Particle examination was acceptable.

Note 3: ,

l No ultrasonic examination was performed downstream due to the proximity of a pipe clamp or upstream due to the j valve configuration. The Magnetic Particle examination was limited to 55.88% of the required examination area. I Due to the complexity associated with hanger removal / reinstallation, a relief request will be submitted prior to RF-6 l to accept the limited examination.

Note 4:

The Magnetic Particle examination was limited to 27% of the required examination area due to proximity of the pipe  ;

support configuration. A relief request will be submitted prior to RF-6 to accept the limited examination.  !

examinations were performed include: Extraction Steam (ES), Feedwater (FW), Gland Steam (GS), Heater Drains (HD), Main Steam (MS) , Off-Gas (OG), and Turbine Generator Miscellaneous Vents and Drains (TD).

Note 1:

Portions of piping were replaced with CrMo material via MWR D62127.

Note 2:

Sections of GSAS and GSA7 piping were replaced with carbon steel via MWR D62748.

Note 3:

A portion of this line was replaced with CrMo material via MWR D62747.

Note 4:

These condenser pipe stubs were noted to be leaking due to steam impingement prior to RF-5. These pipe stubs were replaced with stainless steel material via MWR's D50460 and D52178.

Note 5:

Each of these fittings have been inspected five previous outages. The inspection results show that wear is not a significant problem. These fittings were deferred to allow inspections of new locations in other systems.

Abstract of Examinations: Listing of Substituted and Additional Examinations for Erosion / Corrosion A pinhole steam leak was noted at the toe of the weld at unreinforced tee connection GSTEE4.1 before RF-5, as documented by MWR DS9884 and NSED letters Y-104577 and Y-104584. During RF-5, UTinspections were performed on GSTEE4.1 and 4 other items (listed in Table 7) which represented 2 items of similar configuration and 2 items on the same line as GSTEE4.1.

GSTEE4.1 was found to be badly degraded and was replaced with a reinforced tee connection and straight pipe both made from stainless steel via MWR D59884. GSELL4.7 was noted to have some wear at the extrados and was replaced with stainless steel via MWR D59884 since this elbow is attached to the replacements for GSTEE4.7.

Similarly,1ES16AB was noted to have slight wear at its tee connection and was replaced with a reinforced tee made of stainless steel via MWR D59884.

REPAIR AND REPLACEMENT

SUMMARY

1(]m NIS-2 OWNER'S DATA REPORTS l This section provides a listing of the 57 ASME Section XI repairs or replacements performed between December

! 10,1993 and April 29,1995.

I All repairs and replacements are considered to be of a routine nature. There were no flaws identified during

_ performance of any ISI examinal'on that required a repair or replacement to be performed.

Code Case N-416-1 was utilized during the replacement of valve 1SX173A. This Code Case allowed the elimination of the hydrostatic testing requirement following welded replacement provided a surface examination is performed on the root pass of the pressure retaining welds.

Several modifications were completed during this period, including the following:

. Modifications RT-031, M-064, M-082, and FW-035 consisted of various valve internal component changes.

. ECNs 28818,28821,28822,28826, and 28828 allowed the installation of leak rate testing taps on relief valve flanges.

Snubber reduction was performed in accordance with M-071 and M-073.

. Work was completed on RT-030, which involved piping changes.

Following the listing of repairs and replacements are copies of NIS-2 Owner's Data Reports for all of the Class 1 and 2 repairs and replacements performed during this reporting period. Data reports for any components or parts used in support of the NIS-2's have not been included with the NIS-2 copies but are filed with Section XI repair and replacement records. '

The records for all Section XI repairs and replacements are available and located in the Clinton Records Vault filed in the respective N-5 system under record type 4E.230 entitled "N-5 Data Report Package Supplements".

7. Description of Work Replaced valve to assure successful completion of LLRT test. After installation, the LLRT was successfutty completed but the valve did not operate correctly. The valve was declared inoperable and will be reworked in RF 5 on MWR 056901. The Code replacement performed on this NWR is considered satisfactory.
